Occupational Background History: Christian is a 21 month old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Cristina, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits.

Occupational Background Summary: Christian demonstrates difficulty with fine motor skills due to limitations with object manipulation, manual dexterity and manual control evident by premature grasp patterns, spillage and difficulty with small objects limiting fine motor precision. Christian demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Christian demonstrates moderate delays hindering executive functioning skills, attention, functional behavior and safety. Assessment of Goals/Response to Treatment: The patient presented as friendly and cooperative, demonstrating positive engagement throughout the session. Skilled occupational therapy intervention utilized therapeutic puzzles to address deficits in psychosocial, cognitive, and motor domains. The session focused on improving sustained attention, problem-solving, and eye-hand coordination to support functional task completion. Progress is being made toward the goal of independently initiating non-preferred tasks. During today's session, the patient required moderate verbal and physical cueing to transition to a challenging puzzle. He initiated the task after 6 verbal prompts and brief hand-over-hand assistance, which represents a notable improvement from his baseline of requiring maximal assistance accompanied by adverse behaviors. Furthermore, the patient demonstrated significant gains toward his goal of social participation. He successfully engaged in a turn-taking puzzle activity with a peer for a duration of 3 minutes, requiring only 4 verbal cues to facilitate appropriate sharing and social reciprocity. This performance indicates substantial progress, as the patient previously required total assistance to manage self-isolation and aggressive behaviors during peer interactions. Continued skilled OT is medically necessary to build upon these gains, further reduce cues for task initiation, and increase the duration of positive peer engagement.

11-20-2025 11:18 AMChristian DuranChristian (Prim) Replied: Good morning Saul I could tell you got upset with me wanting Lupita as Christian's OT I didn't mean to get you upset or to think that I want you to do whatever I want all I want now is to know that the only reason why I wanted her to be it's because I already know how she works and how my kids are with her at this time I didn't want to experiment and see how things would work out with someone else since Mateo didn't do much progress with his first OT and then there was Fernandas therapist that she didn't have much confianza she never really wanted to go with him not saying that they weren't good therapist because they were good they tried to help. I hope I didn't cause you any trouble but I don't want to come to my kids therapy's and make anyone uncomfortable and also want to apologize for coming out as demanding that wasn't my intention.

Also being that Lupita already sees Mateo and Fernanda I suggest another therapist to help make scheduling easier as well as to offer a new perspective to therapy as each therapist has their expertise. Lastly, I thought it would be a great idea to have another therapist so that Lupita and the other therapist could do some one on one socials with the siblings while maintaining the one to one therapeutic relationship.
