Child did not experience any complications at birth, does not have any visual or hearing deficits. Child was born with both a lip tie and tongue which were released before 6 months of age. During these activities, he successfully sequenced peg patterns and sorted puzzle pieces by color with minimal assistance, indicating emerging problem-solving and visual processing skills. Focus was placed on developing fine motor coordination to support pre-academic participation. While engaging with Play-Doh, the patient practiced in-hand manipulation and strengthening. When presented with a coloring task, he utilized a developing three-jaw chuck grasp with moderate assistance to remain within 1-inch boundaries for 2 of 5 trials. So happy you are all back! I will be the therapist conducting Leo's feeding evaluation. I’d like to move forward with scheduling his appointment. The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. Alternatively, I can email you the documents in advance so you can complete them ahead of time and bring them with you on the day of the evaluation. Let me know what you prefer (: For this upcoming week, I currently have an opening on Tuesday at 9:30 PM and Wednesday at 12:00 PM. Please let me know if any. of these times work for you. I look forward to seeing you both again!

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.
