Speech Background History: Everly is a 2 year, 3 month old female referred for a speech-language evaluation due to concerns regarding limited communication skills. Per parent report, pregnancy was significant for gestational diabetes, and Everly required routine glucose monitoring following birth to ensure blood glucose levels were within typical limits. Everly was born full term via natural delivery. Mother reported difficulty with milk production postpartum, which resulted in it taking some time for Everly to receive an adequate milk supply. Recently, Everly required blood work due to abnormal laboratory levels; results are currently pending from her primary care physician. Developmentally, mother reported that all milestones were delayed. Everly previously received feeding therapy services at this facility. Mother expressed ongoing feeding concerns and is currently considering reinitiating feeding therapy due to Everly frequently refusing lunch and dinner meals. Everly currently attends daycare full day. Mother reported that Everly is exposed to both English and Spanish in the home environment; however, English is heard more frequently and was determined to be Everly’s primary language. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.
