Speech Background History: Felix is a 4-year, 4-month-old male who was seen on the above date for an initial evaluation per referral of his primary care provider to determine his current level of functioning and whether skilled speech-language intervention is warranted at this time. He has a medical diagnosis of Developmental Disorder of Speech and Language, Unspecified (ICD-10: F80.9). Felix lives with his family, and the primary languages spoken in the home are English. Due to the home language environment, all testing was conducted in English by a bilingual Speech-Language Pathologist. There is a family history of language delay in his twin sisters, which was associated with prematurity. If Felix is deemed appropriate for skilled speech and language intervention, it is recommended that therapy be conducted in English. Felix was accompanied to the initial evaluation by his father, who provided written and oral information referenced in this report.
