Speech Background History: Dawson is a 2-year, 1-month old male who was seen at New Beginnings Pediatrics on October 28th, 2025 from 4:00pm to 5:00pm for an initial speech and language evaluation. Dawson was accompanied by his mother who served as his primary informant regarding Dawson's medical history. Parents report there is a history of speech and language delays within the family. Dawson was referred due to parental and physician concerns with his receptive and expressive language skills. Currently, Dawson produces approximately 30-40 words (home, daddy, yeah, ball). He is reported to have an increase word and animal/environmental noise imitation. Dawson previously received speech therapy services for approximately six months before transitioning to this clinic. The change in service location was initiated at the parents’ request to continue therapy with the current provider. There are no concerns with fluency, voice, or feeding at this time. Dawson's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: There is no significant medical history or chronic conditions. Dawson was born at 38 weeks’ gestation via vaginal delivery, weighing 7 pounds 1 ounce, with no reported complications or perinatal concerns. Dawson's vision is within normal limits (WNL), with no current concerns or impairments noted. Dawson’s hearing is within normal limits (WNL) with no concerns noted at this time. He previously experienced chronic ear infections and received tympanostomy tubes on October 27th to address this issue. Hearing will continue to be monitored, but no signs of hearing loss are currently present. There is no history of allergies. No other surgical procedures or hospitalizations are reported in his history.

Dawson demonstrates emerging pragmatic skills, showing appropriate eye contact and engagement during social interactions- actively engaging in formal and informal social exchanges; however, exhibits fleeting attention to both verbal and non-verbal cues. He requires frequent verbal, gestural, and occasional tactile prompts to respond to basic commands.
Speech Diagnosis/Impressions: Dawson is a 2-year, 1-month-old male who is demonstrating a Moderate-Severe Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Dawson's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Dawson receives services 2x/week for up to 60-minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Dawson is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Dawson's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Dawson is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, long-term services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Speech Progress to Date: The prognosis with therapy is good, prognosis without therapy is guarded. Dawson will be discharged from speech therapy when all of his goals are met and he receives a standard score above -1.5 standard deviations for all impairments, indicating that his expressive and receptive language abilities are within the mild or average range. The parents/caregivers will be responsible for incorporating his Home Management Intervention Program at home. The frequency of therapy sessions will be 2x/week for 30-45 minutes.

Dawson imitated in ~30-35% of opportunities with verbal approximation. Observed imitations include but are not limited: help, no, yes/yeah, colors, animals, etc. with verbal approximation. He continues to IND sign 'please, help, thank you, more,' and verbalize 'more, help, please, all done' with approximation with and without prompts. He was observed to carryover novel phrases 'no mom, where it go, what's that, found it' etc. with verbal approximation + sign. Within 6 months, Dawson will increase his ability to identify and label a variety of developmentally appropriate nouns and verbs with 30% accuracy across 3 consecutive sessions, improving his receptive and expressive vocabulary skills, as measured by therapist collected data and observations. - During play-based activities, clinician labeled many pictured and tangible nouns. Dawson attended to most models and was observed to imitate in ~25% of opportunities with verbal approximation. He IND labeled 'bubble, sticker, ball, spider-man, car,' etc with verbal approximation. Within 6 months, Dawson will produce a variety of C-V combinations (CVC, CVCV, CVCVC) using age appropriate consonants with 30% accuracy across 3 consecutive sessions, improving his overall speech sound production skills, as measured by therapist collected data and observations. - Target phonemes /p, b, m, n, d, t/ within spontaneous semi-structured and play-based activities. Dawson attended to most models with MIN imitation and carryover. He was observed to produce /b, d/ within isolation given MAX multi-sensory input.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. His re-eval is on 3/30 which gives us time!

03-26-2026 11:04 AM	Rachael Haduck	Occupational therapy works on fine motor skills (ADLs)- eating, bathing, grooming, sensory processing, dressing, emotional regulation. When he has his evaluation one of two things will happen: he will either qualify (1-2x week, ~60min sessions) or not qualify. Does that make sense? Do you have any questions about this?

It doesn’t target social interaction as well as sometimes the COTAs will work together in a room so he may get to be with 1-2 other kids (any age). Since they don’t really target speech, he may just naturally pick up on words, phrases, interactions, etc!

03-26-2026 11:18 AM	Dawson Toth	Dawson (Prim) Replied: Yeah that makes sense. I think the reason they're suggesting it is because a lot of the milestones on the papers are speech related. It asked like one or two about how he acts around other kids and if he's interested in them and he is curious when we go to the park or something but I don't think he knows how to go to try and interact with them

He also might be hesitant to interact due to his verbal output, he may/might understand someone outside of the home and therapy might not understand him and what he’s trying to communicate. It could be a bit “fearful” for him. It’s worth getting him evaluated and seeing if he qualifies :) Another great thing in the meantime are a few of things I’ve mentioned in session like activities on post at the library, local events, even trialing daycare 1x/week for a few hours :) These things will really push him out of his comfort zone and expose him to different levels of communication- 5yr old vs 2yr old. :)
