Speech Background History: Noah is a 2 year 3 month old male who was referred for a speech and language evaluation by his pediatrician and parents’ concerns regarding speech delay. Noah was accompanied by both his mothers, and both served as the informants. According to Ms. Guzman, who is the main caregiver, Noah has suffered numerous illnesses and had been diagnosed with torticollis and tongue and upper lip ties when he was 3 months old. Since that time, the tongue and lip ties were released and Noah began home health services for his occupational, physical and speech therapy needs. However, mothers were not pleased with the speech services thus the basis for the evaluation today. According to Ms. Guzman, Noah began using at least 4 words (mama, tortilla, tu, no), but suddenly stopped but, mothers could not recall the date. Since then, Noah has communicated via open mouth vowelizations, grunts, taking/grabbing item of desire, and just beginning to point to the item of interest. Both mothers are concerned with Noah’s developmental delays, Noah was evaluated by Dr. Fonseca recently who found β€œno signs of autism.” However, the family is seeking a second opinion from Dr. Schuttes, as well as a neurologist next month. Per parent report, Noah suffered from severe separation anxiety, has difficulties sleeping and remaining asleep, which has led to frequent night terrors and/or waking up crying as if in pain. Based on clinical observations, Noah’s current communication was characterized as open mouth verbalizations, smiles with agreement, use of babbling and jargon during play, and reaching/taking objects of desire to express his communication wants/needs. A typically developing child that is Noah’s age should have 25 to 100 words in their expressive vocabulary and use 1 word utterances consistently with 2 words emerging. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Noah’s receptive and expressive language skills. Based on Noah’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Noah was diagnosed with torticollis at 2 months of age, and shortly after that he was found to have an upper lip and tongue tie. The ties were released immediately due to the bottle-feeding issues Noah displayed. According to parents, they followed through with the recommended stretches to prevent regrowth. Parents claim Noah has suffered multiple illness ranging from upper respiratory infections, COVID, to the flu. Noah is on multiple medications to treat and prevent the reoccurrence of another upper respiratory infection.

Feeding Background History: Noah is a 2-year 4-month-old male who was referred by his pediatrician for a clinical feeding evaluation due to feeding difficulties and concerns. Noah’s mother served as the informant for the evaluation. Noah’s mother reported and provided documentation that Noah was diagnosed with autism (level 3), attention-deficit/hyperactivity disorder (ADHD-combined presentation), unspecified disruptive behavior disorder, and a global developmental delay by Dr. James Schutte in November 2025. Noah’s mother reported feeding concerns began around 6 months of age when transitioning from a bottle to purees. She reported that Noah underwent a frenectomy (lingual and labial) at 6 months old. His mother reported that he transitioned from puree to solids around 11 months old. Noah’s mother reported current concerns with Noah overstuffing foods resulting in him spitting out foods often. Additionally, she reported that he does not know how to utilize utensils, will not sit in highchair or at table during mealtimes, and will often throw food on the floor and eat it off the floor. Noah is currently receiving OT and PT at another clinic and ST at New Beginnings. Noah’s mother reported that he has previously received feeding therapy since he was a baby. Noah’s mother reported that his pediatrician does not have concerns regarding nutrition/growth.
Feeding Medical History: Noah’s mother reported no concerns with pregnancy or birth. She reported that Noah underwent a frenectomy (lingual and labial) at 6 months old. Noah’s mother reported that there are no know medical allergies, however, Noah does have a pineapple allergy. Additionally, Noah’s mother reported that he was born with a milk and soy allergy, however, has since been resolved. 2.wife had preaclamsia no NICU.
3.He was diagnosed with torticolls.
4. Crawled at almost 1, walked at 16 months, still has trouble feeding himself, stuffs his mouth and won't use utensils.
5.He has a lot of times when he don't sleep wakes up crying and yelling at night will last hours awake.its rare when he sleeps 8 hours. He snores and sleeps with mouth open.
6. He stays him with me no school or day care. He has in home therapy for OT and PT. Since he was like 6 months old. He was also receiving speech but we are not happy with that part.
7.mostly English and some Spanish.
8.not that we know of.
9.we have concern about him not being able to talk. And his speech regressions. He has been through a lot of test and therapist and still nothing. We are thinking alot of his meltdowns and behavioral issues come from not being able to express himself.

Noah has been diagnosed with a severe to profound mixed receptive-expressive speech disorder, where his expressive language skills are more severe than his receptive language skills. Noah would greatly benefit from speech therapy services that address the following: responding to his name, responding to "no" and "wait," following one step commands, relational play, imitating environmental/nonΒ¬speech sounds, and imitating one to two syllable words. Once all paperwork has been submitted, signed and authorization has been approved, the treating therapist will reach out to you to begin speech therapy services.
