Assessment of Goals/Response to Treatment: Sarai actively participated in today’s skilled occupational therapy session, which emphasized improving self-regulation to support engagement in functional cognitive and visual-motor activities. The session began with a therapist-guided sensorimotor activity using “fake snow” to target tactile processing and regulation. With maximal verbal encouragement and therapist modeling, Sarai interacted with the unfamiliar texture using tools, showing early progress in tolerating non-preferred sensory input without signs of avoidance. Following the sensory activity, Sarai transitioned to a structured tabletop task. She was able to maintain attention for approximately four minutes with maximal support for regulation and sustained focus, demonstrating emerging progress toward her goal of engaging for eight minutes. During a visual-motor integration activity, Sarai worked on writing 10 sight words. This required maximal assistance, including hand-over-hand support for accurate letter formation and maximal verbal cues to help with sequencing letters for correct spelling. On standardized testing, patient scored at an age equivalence of less than 4 years old on the BOT-2, indicating well-below-average performance across all gross motor subdomains. These findings are consistent with substantial delays in gross motor development and demonstrate a severe impact on functional mobility and age-appropriate participation with peers. 03-16-2026Guadalupe KeyChild reported to the therapist today that she is being bullied at school by both her teacher and other students. She stated that the teacher holds her hands down, yells at her, tells her she hates her, and refuses to help her. The child also reported that other students bother her and call her “dumb” because she struggles with her schoolwork. Mother was informed of the child’s statements and reported that she is aware of the situation and has already addressed these concerns with the teacher and the principal. However, according to the mother, both the teacher and principal deny the allegations and have not taken any action. Mother explained that the child missed half of the school year due to hospitalization and is currently behind academically and unable to read, which she believes is contributing to the bullying by peers. Mother was provided education on ways to address these concerns with the school district and was advised to document all communications and incidents in writing, as the allegations are serious and should be addressed appropriately. Mother was also educated about IEPs and 504 plans. According to the mother, the school has told her multiple times that IEP services can only be implemented for 45 minutes and cannot be extended. Mother further stated that she has provided documentation of the child’s Autism diagnosis to the school, but they have refused to include it in the child’s academic file. Mother was informed about other school districts that may better support the child’s needs and was encouraged to report and document all incidents that have occurred. Parents also reported that the child is currently being weaned off some medications after being hospitalized due to complications related to medication. The therapist observed that the child’s hands were shaking significantly while completing tasks and informed the parents. Parents stated that the shaking began about a week ago when the medication tapering started. They also reported that the doctor has stated that the child is “a handful” and that it is difficult to keep track of her condition. Mother was informed of other physicians who specialize in working with children with Autism and was advised that she has the right to report the doctor, as such statements are inappropriate

Therapist attempted to call and sent 2 messages to follow up and see if child is out of the hospital or if visits for the week need to be canceled and no response. First week was no call no show, second week child was in the hospital.

The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. Alternatively, I can email you the documents in advance so you can complete them ahead of time and bring them with you on the day of the evaluation. Please let me know what you would prefer. La próxima semana, ¿sería posible que Sarai asista lunes, martes y miércoles a las 3:00 pm? Esta semana no pude verla y eso puede afectar su porcentaje de asistencia. Es importante mantener al menos un 90% de asistencia para poder continuar con los servicios.

También les recuerdo la importancia de avisar con anticipación cuando no puedan asistir a terapia. Cuando no recibimos aviso previo, se considera “no call, no show”, y después de 3 ausencias de este tipo, lamentablemente tenemos que dar de baja al paciente.
