Speech Assessment Summary: Desmond is a 1-year 4-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to language concerns. Desmond’s mother served as the informant for the evaluation. Desmond’s mother reported that he does not consistently use words, however, has heard him attempt to say “mama” and “dada”. She reported an increase in babbling and stated that he will try and imitate sounds and words. She reported no concerns with receptive language at this time. Desmond attended the evaluation with his mother. Desmond appeared shy initially, however, shortly began to engage with the therapist. Desmond demonstrated communicative intent with his mother and the therapist throughout the session. Desmond and the therapist played with a cause-and-effect toy. Desmond was presented with modeling of toy function and was engaged, however, would not initiate play. Engagement in functional, relational play, and/or self-directed play was not directly observed. Desmond was observed to babble in the session. Desmond’s mother reported that he produces vowels and early emerging consonants consisting of /b/, /p/, and /m/. Desmond was not observed to use any words or approximation during the evaluation. Additionally, Desmond did not imitate any sounds, gestures, or words. The Preschool Language Scales-Fifth Edition (PLS-5) was administered to determine Desmond's receptive and expressive language development. Desmond’s Auditory Comprehension Standard Score is 78 (Raw Score 14), which is 1.5 standard deviations below the mean when compared to age matched peers, indicating a moderate receptive language disorder. Desmond’s Expressive Communication Standard Score is 81 (Raw Score 16), which is more than 1 standard deviation below the mean when compared to age matched peers, indicating a mild expressive language disorder. Desmond’s Total Language Standard Score is 78 (Raw Score 30), which is 1.5 standard deviations below the mean when compared to age matched peers, indicating a moderate mixed receptive/expressive language disorder. Receptive language strengths include: interrupting activity when you call his name, looks at objects the caregiver points to and names, and responding to inhibitory words. Receptive language weaknesses include: does not yet demonstrate understanding a specific word or phrase without gestural cues, demonstrate functional, relational play, and self-directed play, following routine and familiar directions with gestural cues, and identifying familiar objects. Expressive language strengths include: seeks attention from others, vocalizes two different vowel sounds, combines sounds, takes multiple turns vocalizing, plays simple games with another while using appropriate eye contact, vocalizes two different consonant sounds, babbles two syllables together, and uses a representational (symbolic) gesture. Fine motor skills involved the small muscles of the body that enable such functions as manipulating daily items and grasping small objects. Weaknesses in fine motor skills can affect a child's ability to eat, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Assessment of Goals/Response to Treatment: Patient was an active and willing participant in today's session, demonstrating positive engagement throughout all therapeutic activities. Progress was noted across multiple goal areas, facilitated through structured and cooperative play. To address self-care skills, a simulated feeding task using Play-Doh and a spoon was utilized. Patient demonstrated significant gains, now requiring moderate physical assistance to scoop and initiate the motor plan for bringing the spoon towards the mouth, a notable improvement from the maximal assistance required at baseline. During fine motor tasks involving a pegboard and puzzles, patient showed progress toward targeted release of objects into a large container, now requiring moderate to maximal assistance for 2 out of 5 opportunities, indicating improved motor control. Furthermore, patient is beginning to transition from a palmar grasp to an emerging radial digital grasp with moderate tactile cues and hand-over-hand assistance for 2 out of 5 trials when manipulating puzzle pieces, which supports improved in-hand manipulation. Improvement was also noted in sensory processing and functional play skills; during sensorimotor play, patient independently explored toys with hands for approximately 35-40% of the activity with moderate verbal redirection from mouthing, demonstrating clear progress toward the goal of 50%. Cognitive and psychosocial skills such as sustained attention, sequencing, and problem-solving were supported through board games and sorting activities, with patient successfully initiating and terminating tasks with minimal prompting.

At this time, we need a copy of your child’s Medicaid care plan insurance card. Please note that we are no longer accepting the traditional Medicaid card as valid insurance on file. We require the plan-specific Medicaid insurance card, such as (Superior, Molina, El Paso Health, Wellpoint, TMHP) If you do not have the physical card or are having trouble locating it, most Medicaid plans offer an online portal where you can access a digital E-card.
