Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection.

Assessment of Goals/Response to Treatment: Brayden presented to session A&O and cooperative, demo’ing excellent engagement and motivation throughout all therapeutic activities. Skilled OT services were provided ADLs, sensory processing, VMI, attention, and psychosocial skills through structured and graded interventions. During UBD, Brayden demo’ed progress by I doffing pullover shirt and req. min VC for orientation to loop head through during donning, indicating improved body awareness, sequencing, and motor planning. Sensory integration was targeted through sensorimotor play using shaving cream, where Brayden initially req. mod A and VC to engage c novel tactile input but was able to sustain participation in 1–2 min intervals totaling ~10 min, reflecting increased tolerance to tactile stimuli and improved sustained attention. VMI and psychosocial skills were addressed via coloring activity, during which Brayden demo’ed appropriate help-seeking by requesting A to open markers and improved impulse control by waiting c min VC, supporting development of social-pragmatic skills and self-regulation. Session concluded c functional handwashing task, where Brayden completed all steps c min VC for transitions, indicating gains in cognitive integration, sequencing, and ADL I. Overall, Brayden is demo'ing measurable progress in ADLs, sensory tolerance, attention, and psychosocial skills; however, continued deficits in UBD I, tactile processing, and consistent task sequencing persist, and he continues to benefit from skilled OT services to further improve functional I and participation in age-appropriate routines.

To stay ahead of this change and help prevent any interruptions in your child’s therapy, we would like to know how you would like to proceed. The options to proceed would be to remain with Dr. Patel and switch to a different insurance plan, or remain with Superior Health and transition to a different pediatrician. If you choose to stay with Dr. Patel and switch insurance plans, we will need a new referral for all services under the new insurance, as well as a copy of your new insurance ID card so we can verify benefits. If you choose to switch pediatricians and remain with Superior Health, we will need a new referral for all services from the new primary care provider (PCP). Please contact the front office as soon as possible so we can help coordinate this change and ensure there are no gaps in your child’s therapy services. Thank you for allowing us to serve your child!

We understand that timely care is essential. To help avoid further delay, we want to offer you early access to therapy via telehealth services. This option will get your child's therapy started now, while you wait for in-person therapy. Our telehealth sessions are conducted securely via Zoom, allowing your child to receive high-quality, personalized OT from the comfort of your home. Once your child's in-person spot becomes available, you will have the option to come into the clinic for therapy or, if you prefer, continue with telehealth if your child is a good candidate. Please let us know if you are interested in starting early or just waiting in line for in-clinic therapy. Thank you!

Yes to medical diagnosis (Autism) 3) Delayed developmental milestones 4) No sleeping or breathing issues, no snoring. Sleeps with mouth closed, no enlarged tonsils. He does sleep 8-10 hours on a regular basis. 5) No medications except for multivitamins and omega 3 6) He's in kindergarten 7) No hearing issues. He does wear glasses 8) My concerns are that he's nonverbal, he has difficulty understanding directions and he requires support for every day life activities 9) My concerns began when he was 1yr 1/2 old 10) Yes he had been attending OT and speech therapy for a little over two years now at OT for TOTS

Brayden was rushed to the NICU department, was given antibiotics and had to remain there for 7 days, during those days he also develop jaundice. He was diagnosed by Dr. jignesh Patel. He's currently attending North Loop International School And Yes he does have an ACC device that was provided by his previous therapist

02-12-2026 03:57 PM	Daniel Valdivia	Hi, I wanted to give you an update on how he did today. I wasn’t able to catch you after his speech appointment, but he had a really great session! He completed a coloring activity and remained seated for the entire task. He also participated in a coin piggy bank activity, where he did a nice job taking turns with me. In addition, we worked on tabletop sorting activities using wooden pieces by color and shape. He did very well with color sorting and just needed a little assistance with identifying the shapes. I’ve also noticed great progress with handwashing. He is completing more of the steps independently and now mainly needs occasional verbal prompts rather than physical assistance. At the end, I gave him the last 5–7 minutes in the gym as a reward for how well he did! Overall really good day today!

Within 3 months, Brayden will independently complete all steps of handwashing (turning on water, wetting hands, applying soap, scrubbing 20 seconds, rinsing, turning off water, drying hands) with moderate assistance in 3/5 opportunities to increase independence with self-care skills

Because of holidays and a few sick days, Brayden has fallen behind by about 10 sessions. These things happen, but it does mean it’s important for us to try to make up some of those missed visits (at least 5-7 if able) until his re-evaluation in April so we stay within the insurance expectations. :)

Just to reiterate, Brayden is currently approved for 52 therapy sessions within a 6-month authorization period. Insurance companies typically like to see around 90% attendance, which would be about 47 sessions out of the 52. Right now he is at 43 (out of 52), which is great ( I forgot to mention that initially, sorry)! To clarify yesterdays message because I understand it was confusing- prior to Friday (last week) he was at 11 sessions, however, because we able to complete that make-up session it reduce him to 10. :) The main reason he currently has more remaining sessions is exactly what you mentioned—the holidays and then being sick, which is completely understandable. I just like to stay on the safe side with insurance, since Texas can sometimes be strict about coverage and attendance requirements, I never want to see any one of patients be at risk of reduced sessions. So in short, he just needs to complete at least 4-5 more sessions to be at that 90% (47/52 sessions)! :) I really hope all of that makes sense. Sometimes it's difficult making sure I'm covering exactly what I mean to convey but he is on the right track! And if you continue to be comfortable with it, Lauryn is here to help with make-ups if I do not have the availability, are out sick, and/or are not available on a Friday :) thank you again!!!
