Occupational Background History: Adaleth is a 3.9 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Thannya, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 32 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression. Mother reports child's functional behavior impedes functional skills , ADLs and engagement in daily roles and routines. Mother expressed that child will hit others, throw objects, break things, and engage in self harming behaviors when frustrated. During the evaluation child was observed hitting mother and throwing objects upon non preferred task. Adaleth will take off her shoes and throw the, fall to floor and scream.

Occupational Background Summary: Adaleth demos difficulty with fine motor skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Adaleth demonstrates moderate delays hindering executive functioning skills, functional behavior and safety. Child also demonstrates difficulty with manual control skills, object manipulation, manual dexterity hindering fine motor precision and accuracy. Child will stop task due to difficulty processing what was asked limiting engagement. Child also demonstrates fine motor and grasp delays in BUE evident by premature grasp patterns, spillage and difficulty with manual dexterity tasks. Adaleth also demonstrates deficits in sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child demonstrate medical necessity deficits that warrant skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Child to greatly benefit from continued skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Assessment of Goals/Response to Treatment: The patient presented to the session alert and oriented, demonstrating full engagement and completion of all structured therapeutic activities. Skilled intervention focused on cognitive integration, psychosocial skills, and motor skill development. Significant progress was noted in sustained attention; whereas the patient's baseline was 15-20 seconds of attention with maximum assistance, today the patient was able to sustain attention for up to 35 seconds on a structured fine motor task with moderate verbal and tactile cueing. This demonstrates measurable progress toward the goal of achieving one minute of sustained attention. Challenges with emotional regulation and activity initiation were present at the start of a social activity. The patient demonstrated defiant behaviors for approximately 15 minutes prior to engaging in a peer-based task. Through the implementation of skilled co-regulation strategies and the provision of structured choices, the patient was able to de-escalate and successfully initiate turn-taking and positive social reciprocity with a peer. This indicates a continued need for intervention to improve coping skills and self-control to facilitate smoother transitions and task initiation.

To help avoid further delay, we want to offer you early access to therapy via telehealth services.
This option will get your child's therapy started now, while you wait for in-person therapy. Our telehealth sessions are conducted securely via Zoom, allowing your child to receive high-quality, personalized OT from the comfort of your home.
Once your child's in-person spot becomes available, you will have the option to come into the clinic for therapy or, if you prefer, continue with telehealth if your child is a good candidate.
Please let us know if you are interested in starting early or just waiting in line for in-clinic therapy.
