Speech Background History: Zekiel Luera is a 2-year 8-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Zekiel’s mother served as the informant for the evaluation. The evaluation was performed in English, as this is the primary language at home. Mother reported Zekiel was born at 39 weeks with no complications. Mother reported a slight vitamin C deficiency however no concerns and no NICU stay. Mother reported all developmental milestones were met within average range with the expectation of speech. Mother reported Zekiel was crawling around and hit his head however no concussion was reported by PCP. Mother reported Zekiel currently stays at home with mother and are currently considering preschool.. Mother is concerned as Zekiel has very limited language and is very difficult to understand. Zekiel is now starting to hurt himself. Pediatrician recommended getting tested through a developmental specialist. Zekiel received services through ECI.

He participated in turn-taking in several opportunities given maintained support. Specifically, his Grasping subtest standard score of 4 (2nd percentile) reflects an age equivalency of 13 months, a 24-month delay, and his Visual Motor Integration subtest standard score of 2 (less than 1st percentile) reflects an age equivalency of 10 months, a 27-month delay. Assessment of Goals/Response to Treatment: Zekiel presented to session A&O c friendly, cooperative demeanor, demo’ing excellent readiness for therapeutic activities. Skilled OT services were provided to address self-regulation, cognitive integration, ADLs, FM, and psychosocial skills through structured and sensorimotor interventions. To support regulation, Zekiel tolerated proprioceptive input via crash pad, facilitating a more organized state for task engagement. Zekiel demo’ed emerging sustained attention, maintaining engagement in 1 FM task (Squigz on vertical surface) for >3 min c min VC; however, attention to 2 subsequent tasks decreased to ~1 min each before req. redirection, indicating inconsistent task persistence. Progress was noted in psychosocial skills, as Zekiel engaged in reciprocal ball play for 5 consecutive turns c therapist prior to I termination, reflecting improved social reciprocity and engagement. ADL skills were addressed through simulated feeding task, where Zekiel completed 10 scoops using spoon; however, he req. max HOHA for all trials to facilitate appropriate grasp and motor planning, indicating significant deficits in utensil use, coordination, and functional I. Overall, Zekiel is demo'ing progress in regulation, attention in preferred tasks, and social interaction; however, persistent deficits in sustained attention across tasks, FM coordination, and ADL I continue to impact functional performance, and he continues to benefit from skilled OT services to improve self-regulation, utensil use, and participation in age-appropriate activities in alignment.

Physical Therapy Assessment: Child assessed using a Standardized Assessment Clinical Observations: significant, constant toe walking with and without shoes, weakness, cut on bridge of nose from fall Parental Report: to get him to move like he should and not toe walk Patient demonstrates deficits in gross motor skills. Patient referred to physical therapy due to atypical gait/ toe walking. Patient presents with weakness, decreased focal attention, decreased balance, lacks coordination, atypical gait, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 2 year 9 month old male who scored at 18 month level in Gross Motor area on the PDMS-2 assessment indicating a very poor status with simple and complex skills; thus severely impacting development. Without skilled physical therapy, the patient is at risk for developing further impairments that may negatively impact the ability to perform both simple and complex daily tasks, sustain proper posture, fulfill ADLs , and limit overall quality of life. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Patient referred to physical therapy due to atypical gait/ toe walking. Patient presents with weakness, decreased focal attention, decreased balance, lacks coordination, atypical gait, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 2 year 9 month old male who scored at 18 month level in Gross Motor area on the PDMS-2 assessment indicating a very poor status with simple and complex skills; thus severely impacting development. Without skilled physical therapy, the patient is at risk for developing further impairments that may negatively impact the ability to perform both simple and complex daily tasks, sustain proper posture, fulfill ADLs , and limit overall quality of life. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Mom voice starting the first week of February she would like to go down to 1x/wk for ST and 1x/wk 30 mins for OT. Parent also stated that they're planning on gaining coverage end of March or beginning of April.
01-20-2026	Victoria Chacon	The supervising SLP was notified by the treating SLPA of significant and ongoing safety concerns during speech therapy sessions. The patient presents with severe behavioral dysregulation consistent with suspected Autism Spectrum Disorder, currently undiagnosed. During recent sessions, the patient demonstrated aggressive and self-injurious behaviors, including but not limited to head-butting, hair pulling, hitting, pinching, and attempts to harm himself. These behaviors have resulted in physical injury and ongoing safety risks to both the patient and the treating therapist. The treating SLPA has appropriately implemented a wide range of behavior-regulation and sensory-based strategies within her scope of practice, including use of a larger treatment space, crash mats, weighted vest, sensory toys, dimmed lighting, sensory lighting, oral sensory input (chewy tube), deep pressure/compressions, and movement-based regulation attempts. Despite consistent and appropriate use of these interventions, the patient remains unable to achieve a regulated state sufficient to safely engage in speech or language therapy tasks. At this time, the patient’s behaviors significantly limit therapeutic access to speech-language goals. The treating therapist reports that sessions are focused primarily on maintaining safety rather than addressing communication objectives, and that environmental modifications alone have not resulted in improved regulation. The current treatment setting does not allow for safe physical management without increased risk of injury to both patient and clinician. The patient is currently in the process of being evaluated for Occupational Therapy and Physical Therapy services to address sensory regulation, motor planning, and self-regulation needs. Based on the severity and persistence of unsafe behaviors, the supervising SLP recommends consideration of placing speech therapy services on hold or discharging from speech therapy until the patient has received appropriate OT and/or PT intervention to establish foundational regulation and safety skills necessary for participation in speech-language therapy. Continued speech therapy under current conditions is not clinically appropriate or safe and does not allow the patient to benefit from communication-based intervention at this time. The supervising SLP supports the treating SLPA’s request for additional support and acknowledges that the treating therapist has acted appropriately, professionally, and within scope in managing this case. The treating SLPA has demonstrated sound clinical judgment in recognizing when patient safety and therapeutic effectiveness are compromised. Ongoing exposure to physical aggression places the therapist at risk of injury and burnout, and continuation of services without adequate interdisciplinary support is not recommended. Supervisor has previously observed sessions, and has worked on finding strategies for patient to regulate, however it has progressively gotten worse. Supervisor will observe next session to plan for next step.

01-20-2026	Carla Milan	Patient Note – Behavior/Safety Therapist attempted to engage Zekiel in a structured play activity. Carla Milan's assigned therapy room was unavailable due to use by another assistant; therefore, the session was conducted in the Director’s room. Zekiel became fixated on the light switch and repeatedly attempted to turn the lights on and off. When redirected, Zekiel became dysregulated and upset and attempted to hit his head on the wall. Therapist provided physical support to prevent self-harm. During this time, Zekiel pulled the therapist’s hair. Therapist attempted to redirect Zekiel to the floor to play with blocks and puzzles ; however, Zekiel continued to head-butt the therapist’s chest multiple times, causing the therapist to be pushed back and hit her back into a feeding chair. Therapist attempted calming strategies including rocking, hugging, and deep pressure while seated in the Director’s chair holding Zekiel. Zekiel continued attempting to hit his head on the table and the therapist’s chest. Zekiel pinched the therapist’s face and pulled her hair several times. Despite consistent redirection and calming strategies, Therapist was unsuccessful regulating Zekiel . Due to ongoing safety concerns, therapist was required to carry Zekiel out of the therapy room. The primary focus remained on maintaining safety and preventing injury to both the patient and therapist. Therapist has communicated safety concerns to Clinical Director Stephanie Escobar and Supervisor Tori Chacon, specifically regarding working with Zekiel in the Purple Room due to environmental hazards (cabinets and door latches) that increase risk for head injury despite using two crash mats . Mother was informed of Zekiels behavior . Mother stated " he came in happy but doesnt know what happened" . Therapist informed mother she would be communicating with both the Clinical Director and Supervisor to develop a plan to better support Zekiel and ensure safety during therapy sessions. Therapist also informed mother that it may be beneficial for Zekiel to prioritize Occupational Therapy (OT) . Mother reported that Zekiel’s first OT appointment occurred today and that PT is currently coordinating scheduling to continue services.
01-06-2026	Carla Milan	During today’s session, Ezekiel demonstrated significant uncooperative and aggressive behaviors. He engaged in physical aggression toward the therapist, including hitting and pulling the therapist’s hair. Ezekiel also began head banging behaviors, creating a safety concern. Due to the intensity of behaviors, the therapist requested assistance from the front office to help place mats in order to maintain patient safety. Despite redirection and verbal prompts, Ezekiel continued to display unsafe behaviors. To support regulation and de-escalation, the therapist implemented calming strategies, including carrying Ezekiel and providing linear rocking motions, as well as deep pressure compressions. During the escalation, the therapist was struck in the neck twice when Ezekiel forcefully hit his head against the therapist and continued to pull the therapist’s hair. Safety measures were maintained throughout the session, and the focus remained on supporting Ezekiel’s regulation and preventing injury to both the patient and therapist. When mother was informed of behavior mother stated that he has been acting such way since christmas when he was ill. Therapist recommended occupational therapy again . Mother stated she was 3rd on the waitlist but is still waiting to be called back . Mother also stated she will start physical therapy tomorrow.

11-13-2025	Carla Milan	Zekiel demonstrated difficulty transitioning between activities and exhibited a tantrum when asked to clean up his toy balls. He began hitting, crying, throwing himself to the floor, biting his mother, screaming, and attempting to self-harm by banging his head on the cabinet and walls. Therapist attempted to redirect his behavior by applying deep pressure to his head and providing firm hugs for regulation while rocking him on the floor in a linear motion ( per suggested on Sensory training ) . During the episode, Zekiel attempted to bang his head on the therapist’s chest. The therapist requested the mother’s assistance, and while she was holding him, Zekiel attempted to bite her again. Therapist also used sensory brush to help regulare Zekiel. Throughout the session, Zekiel was observed stimming with his hands, jumping repeatedly, and demonstrating little to no eye contact with the therapist or his mother.Mother expressed concerns and reported that Zekiel previously received Speech, Occupational Therapy, and Physical therapy through ECI, however visits were not consistent as ECI was always rescheduling services. Mother noted that Zekiel is putt objects in his mouth and has been chewing on various items, including pieces of the wall at home. Mother expressed that Zekiel requires constant supervision due to this behavior. She also shared that he struggles with bowel movements, which she believes may be related to his sensory and oral-seeking behaviors. When asked if she had discussed these concern with his pediatrician, mother stated the pediatrician advised to “wait until their next appointment.” Therapist provided sensory toys to support regulation; however, Zekiel placed them in his mouth. Therapist recommended Occupational Therapy to address his sensory and oral-seeking behaviors. Mother reported she has already submitted a referral but was informed by the front office that there is currently a waitlist. She also mentioned having a Physical Therapy referral due to Zekiel walking on his tiptoes, but stated that the link to set up the evaluation was not working. Therapist communicated with the front office, and they confirmed that Occupational therapy has a wait list and they would contact the mother to schedule the initial Physical Therapy evaluation. Mother stated she is willing to follow all recommendations from New Beginnings and is seeking guidance and support to better assist Zekiel. Therapist reassured mother New Beginnings is here to help her. Therapist sent message with concerns regarding self harming behavior to both Supervisor Victoria Chacon asking for direct observation and Director Stephanie Escobar for a larger room with mats to avoid Zekiel from self harming during therapy . Both Supervisor and Director stated they would assist helping.

I tried reaching out via phone call but missed you all, so I wanted to send a message to see if we can start planning for Zekiel's therapy sessions. I currently have a therapist that has Wednesdays from 11am to 11:30am available for occupational therapy than child would transition to speech therapy session with Ms. Stephanie at 11:30 to 12 noon.
