I contacted your PCP’s office this morning to request a new referral using your primary insurance, BCBS. However, they informed me that the only insurance they have on file is Superior.
In order to process the referral, your PCP’s office needs you to call them directly and provide your primary insurance information (BCBS). Once they update your records, they will be able to send us the referral.
Please reach out to your PCP’s office as soon as possible so we can move forward with your care.
Let us know once this has been completed or if you have any questions. |

We would like to inform all patients with Blue Cross Blue Shield (BCBS) insurance that insurance benefits reset at the beginning of the new year. This means that all copays, coinsurance amounts, deductibles, and out-of-pocket maximums will reset.
As a result, payments for visits will be charged at the full allowed amount until your deductible is met. Depending on your specific BCBS plan, you may or may not have a deductible.
Below are the allowed amounts that will apply until your deductible is met:
Speech Therapy (ST):
Initial Evaluation: $181.77
Visits: $68.24
Re-evaluation: $234.19
Feeding Therapy (FD):
Initial Evaluation: $67.73
Visits: $67.73
Re-evaluation: $234.19
Occupational Therapy (OT):
Initial Evaluation: $80.31
Visits: $118.34
Re-evaluation: $55.41
Physical Therapy (PT):
Initial Evaluation: $80.31
Visits: $118.34

Once your deductible has been met, your copay or coinsurance amounts will then apply according to your plan benefits.
