Sebastian's receptive and expressive language WEAKNESSES are as follows: following simple spoken commands, responding to "where" questions, pointing to five or more familiar persons, animals or toys, using a word for parent or caregiver discriminately, spontaneously saying familiar greetings and farewells, using at least 5 words, saying one word that conveys entire thought; meaning depends on context, naming familiar characters or items seen on TV or in movies. During the oral motor assessment, the therapist attempted to evaluate Sebastian for possible tongue and lip ties due to family history, as both of his siblings have confirmed ties. Sebastian did not open his mouth wide enough to allow for a full observation and consistently bit down on the tongue depressor, limiting the examination. However, a heart-shaped tip of the tongue was noted, which can be indicative of a tongue tie. Additionally, a visible gap between the two front teeth with tissue extending between them was observed, suggesting the presence of a lip tie. Mother also reported a history of breastfeeding difficulties, which can be associated with lip tie. The therapist also attempted to visualize the tonsils due to reports of open mouth breathing; however, Sebastian did not open wide enough to allow for adequate visualization. These concerns will be monitored and further assessed at a later date. Based on formal assessment, parent interview, and clinical observation, Sebastian presents with a MILD MIXED RECEPTIVE AND EXPRESSIVE Language Delay. Speech therapy is recommended at this time as Sebastian does not communicate like other children his age. Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.)
