During the previous authorization period, Leonal experienced a fractured arm and was treated at an urgent care facility. This injury resolved without complications and represents the only change in his medical history. Leonal continues to attend school regularly and participates in academic activities within the classroom setting. Leonal is a bilingual child, with Spanish serving as the primary language spoken in the home, while English is the primary language used in his school environment. During both structured testing and informal interactions, Leonal demonstrated a preference for communicating in Spanish. In order to support accurate speech sound development and promote generalization across environments, therapy services are recommended to be provided primarily in Spanish with English support. To assess Leonal’s articulation skills, he was administered the Goldman-Fristoe Test of Articulation-3 Spanish. Assessment of Goals/Response to Treatment: The patient was alert, cooperative, and an active participant throughout the session, successfully completing all therapeutic tasks presented. Progress towards established goals is noted. Parents report consistent and successful implementation of the home management intervention program as part of their daily and weekly routine, which is facilitating the generalization of skills into the home environment. Significant gains were observed in the production of the /s/ phoneme across all word positions. During structured practice, the patient produced /s/ in the initial position of words with 70% accuracy. The patient also achieved 60% accuracy for /s/ in the medial position and 60% accuracy in the final position. 09-26-2025 01:11 PM	Lailani Chehedeh	Hola, Quiero compartir con usted los resultados de la evaluación de lenguaje de Lionel. Él salió muy bien en la parte de lenguaje, que mide cómo entiende y cómo se expresa. Sus resultados estuvieron dentro del rango promedio, así que no hay preocupaciones con sus habilidades de lenguaje en este momento. Sin embargo, notamos que todavía tiene dificultad con algunos sonidos, como la “S”, y cómo los usa en palabras y oraciones. Por esta razón, le recomiendo terapia de lenguaje una vez por semana, por 30 minutos. Sus necesidades no son muy graves, pero trabajar en estos sonidos de manera regular le ayudará a mejorar. Voy a mandar la referencia para Lionel y espero recibir la autorización para las visitas de Lionel y Kylie más o menos al mismo tiempo. Le avisaré tan pronto como la reciba. En cuanto al horario, puedo ver a Lionel los sábados a las 10:00 a.m., o después de Kylie los miércoles a las 7:00 p.m. Por favor dígame cuál opción le acomoda mejor.
