Assessment of Goals/Response to Treatment: Alexis presented as alert, oriented, and cooperative, demo'ing eager participation throughout the skilled occupational therapy session. Intervention focused on enhancing cognitive, motor, and psychosocial skills through a series of structured therapeutic activities. Alexis engaged in a peg toy task to address VMI and grasping skills, successfully manipulating pegs c an emerging pincer grasp. Cognitive integration was challenged through the use of puzzles, where Alexis demo'ed improved problem-solving by I correcting misplaced pieces. Social reciprocity and activity initiation were addressed via parallel play, during which Alexis maintained appropriate shared space and engagement. Significant progress was observed in relation to behavioral goals. When presented c a frustrating puzzle piece, Alexis demo'ed positive coping mechanisms and req. no redirection, resulting in zero instances of head-hitting during the session. Furthermore, Alexis exhibited excellent impulse control during activity termination and transitions, successfully refraining from throwing toys off the table in all 3 out of 3 opportunities provided. Alexis' performance indicates consistent progress toward functional behavioral regulation within a therapeutic setting.

Parent stated they didn't want to continue with therapy due to how well the child did in re-eval. 12-17-2025	Berenice Escobedo	Mother of Alexis has been going back and forward with ST and OT therapists regarding Alexis schedule. Alexis attends head start and we are have coordinated to see him as early as possible since we (Daniel and myself) do not have after school availability. We have explained to mother make up visits and the importance of them, as well as the 90% attendance policy. Father is the one to bring patient in every week, mother is always moving the schedule around. Per mother's request and due to availability I offered a 7:30 time slot that just opened up, had coordinated with Daniel for one day but not possible on the second day. Mother denied these slots, she will be staying on the original schedule for both services.
12-02-2025	Berenice Escobedo	Mother of patient had requested to only discuss make up visits with her. I have been messaging mother about make up visit options, dates and times, and mother does not respond. Father is the one that brings in patient weekly. I asked father if he could please discuss with mother about make up times or days, father requested to do two make up visits this week. He said that he messaged mother to inform her about the dates and times (which he would be bringing the patient). I let him know about our attendance policy, the dates we are closing for the holidays, and the importance of staying on top of attendance. Patient is twice a week and it is very easy to fall behind and so difficult to catch up. Patient is already 4 visits behind and at a 57 % attendance rate.
11-05-2025	Berenice Escobedo	**Per mother's request 11/5/2025 any make up visits need to be coordinated with her only. Father is the person that has been bringing patient to his therapy sessions. According to father he is going to be the one bringing him in all the time. I had coordinated with father make up sessions for the week of Thanksgiving, explained our 90% attendance policy, and explained how make up sessions work. He had agreed to bring patient in for the next two weeks. Patient goes to head start and mother stated that they are very strict with attendance, options for make up visits are limited due to him attending school. Today mother had messaged me in the morning questioning why there was a 6pm visit, I proceeded to explain that I had coordinated with father and that it was a make up session. She gave me father's number so I could call him and remind him about the appointment. I explained to her father was added to the messages so he could communicate and check in as he is the one bringing in the patient. Mother called front office to try and cancel the appointment I had coordinated with father, stating father couldn't make it. Phone wasn't hung up and mother overheard the conversation between front desk and I where I was explaining to front desk about the miscommunication that has been occurring between parents for the past two weeks. Father mentioning he is confused because he receives our end of the conversation but doesn't know what is going on because he cannot see what mother is sending. Mother is arranging all of the appointments but father is the one bringing in the patient. It seems like parents do not have any communication. Per mother's request she only wants me to schedule make ups with her. Patient goes to school from 8-3, availability will be limited.
11-05-2025	Julie Mora	This morning, a parent called requesting to speak with Berenice. Avery informed the parent that Berenice was currently with a patient and offered to take a message. The parent proceeded to cancel today’s scheduled appointment. After the call, Berenice, Avery, and I began discussing ways to improve communication between parents to ensure both are on the same page regarding appointment scheduling. The reason for this discussion was that the child’s father had agreed to the makeup appointment, but the mother called to cancel. Similar situations have occurred with other families, so we were brainstorming potential solutions to bring up at our next team meeting. Unfortunately, we later discovered that the phone had not been properly disconnected, and the parent overheard our internal conversation. The parent subsequently called back, expressed that she was upset about what she heard, and informed Avery that she would be seeking services from another therapy clinic for her child.

I told parent that we will let Berenice know because she was with a patient. Front office was brainstorming with therapist what to do for this issue next time, because this had been a common issue since patient had been getting seen. Call wasn't hung up and parent over heard this conversation. Parent called back and notified front office that she heard us talking about us brainstorming how to better the situation for the office and the parents in this situation and requested to cancel the appointment and she wasn't sure if she wanted to D/C from services. I was able to give him the letter for school, he has it in an envelope. It does specify days and times, I hope that helps. When the patient is sick, or I am out we make up the visits at another available time or day. We are required to stay within the 90% attendance. Due to me being out of town the week of Thanksgiving, I make up my patient's visits with plenty of time in advance. In December the clinic will be closed from Dec.24th - Jan. 1st, so we will also be making up those visits. For days like these when I am out or holidays, I try to open up Fridays or Saturdays to help with make up visits. However, I am already full this week for those days. This is for all services, OT will also be reaching out to schedule make up sessions. If you want to give us maybe available dates and times and we can try our best to schedule him within those times. His regular schedule does not change, this is an additional visit during the week.

Please let me know if either of these would work for you :) Option 1: We can keep his schedule the same, with sessions on Monday and Tuesday at 9:00 AM. I would provide school notes for both days to excuse his absences. Option 2: I can continue to see him on Monday at 9:00 AM, as I don’t currently have afternoon availability that day, and provide a school note for that session. However, I already spoke to Daniel and he only has availability to see Alexis after me (8:00) on Wednesday. There are some options available, I do need a response and confirmation of what you would like to do by this Friday at the latest, please. 1) I could move Alexis to Wednesday and Thursday at 7:30 for speech. He would be seeing Daniel ONLY on Wednesday at 8:00. 2) We could leave the schedule as is, speech 8:30, OT after Monday and Tuesday. 3) You could still see him a Monday or Tuesday as is, and see him Wednesday. This would be just one day of a little late arrival, and Wednesdays he would be done by 9:00. Mr. Daniel gave you the options he has for OT in the afternoon. You can also choose to do 7:30 speech and pick his other times. This would just mean different times for each service and multiple drives to the clinic throughout the week. Whichever option you all choose just let each therapist know please as soon as possible so we can continue to plan accordingly. Thank you so much! 12-18-2025 09:10 AM	Berenice Escobedo	Good morning. Just to clarify and so that we are all on the same page. We are staying in the original schedule, nothing will change. Mondays and Tuesdays speech at 8:30 and OT after at 9:00. Regarding make up visits it is something that we have to complete. We are given by the insurance a specific amount of visits that we are saying we are going to need to work with the patient and help them achieve their short and long term goals, the doctor approves it (just like a prescription). So we need to be at 90% attendance in order to be able to work on our goals and for patient to have the opportunity to make progress. When the patient is absent, sick, or us (therapist) have to out we are expected to make up those visits. We will coordinate with you as best as possible to make them up possibly within that week. We understand because of school it gets complicated, so we will need your commitment to try and bring him at other times that may become available for those make up visits. I will try to bring him in afterschool hours when the slots open due to cancelations. Every therapist works different as we all have different schedules, we cannot coordinate all the time (we do try our best). For speech, Alexis is still pending some make up visits, I will coordinate when we come back in January for making up those days in the afternoon. Thank you!

The address is 1510 N Zaragoza Rd, Suite B1, El Paso, TX 79936. It is very close to the current building, and I’m happy to send a photo showing its location in relation to where we are now if that would be helpful, just let me know! I’ve already checked out the space, and it has a lot of great materials and activities that I’ll be able to use during our sessions. My first official day at the new location will be February 2nd, so please still come to the building you have been until the end of the week.
