Parent consented to physical therapy evaluation today. Upon repeated concern with how patient walking, family went to UMC and found to have fracture in left tibia. Patient would be casted for 6 weeks. Post getting out of cast patient limping, atypical gait. Patient active and participates in soccer. Patient presents with weakness, decreased mobility, decreased balance, lacks coordination, atypical gait, decreased activity tolerance, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 3 year old female (37 months) who scored at 18 month age equivalence in Gross Motor area on the PDMS-2 assessment indicating a poor (stationary) and below average (locomotion) status with simple and complex skills; thus moderately-severely impacting development. Skilled physical therapy services are required to address and promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Assessment: Child assessed using a Standardized Assessment Clinical Observations: weakness, limited dynamic balance Parental Report: to get stronger and have better balance The patient is a 3 year, 7 month-old female referred to skilled physical therapy due to atypical gait pattern, status post left tibia fracture of unknown etiology (April 2025). The patient has participated in skilled physical therapy services since October 14, 2025, and has demonstrated measurable progress toward functional goals. Improvements include resolution of observable limp, decreased frequency of falls, and increased activity tolerance without early fatigue. Currently, the patient continues to present with gross motor delays and impaired balance for age. Standardized assessment using the PDMS-2 indicates persistent deficits, with Stationary skills remaining at an age equivalence of 18 months (no change from prior assessment) and Locomotion improving to a 28-month age equivalence, which remains significantly below chronological age. These findings indicate ongoing impairments in balance, postural control, and higher-level mobility skills. The patient with recently onset of falls per caregiver report possibly due to external factors (sandals with limited foot support) contributing to change in status. The patient exhibits underlying deficits in dynamic balance, coordination, and motor planning that place her at risk for injury and limit safe, independent mobility. With current gross motor delays, functional limitations, and risk for falls, skilled physical therapy remains medically necessary to address deficits in strength, balance, coordination, and age-appropriate motor skill acquisition. Ongoing therapy is required to facilitate progression toward age-appropriate gross motor milestones, improve safety with functional mobility, and reduce fall risk. She has a lot of calf tightness but was able to tolerate the manual therapy really well. She worked on a lot of balance exercises and core strengthening to help her stop falling as much.
