Patient referred to physical therapy due to developmental delay not walking at 16 months. Patient presents with weakness, decreased mobility, decreased balance, lacks coordination, atypical gait, decreased activity tolerance, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 16 month old male who scored at 10 months age equivalence with Body Control and 9 months age equivalence with Body Transport in Gross Motor area on the PDMS-3 assessment indicating a below average status with simple and complex skills; thus moderately- severely impacting development. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved.
