Speech Background History: Anthony is a 12-year, 2-month old male who was seen at New Beginnings Pediatrics on December 1, 2025 from 9:00am to 10:00am for an initial speech and language evaluation. Anthony was accompanied by his mother who served as his primary informant regarding Anthony's medical history. Anthony was referred due to parental and physician concerns with his receptive and expressive language skills. Currently, Anthony produces “mom” verbally but primarily communicates using his schools AAC device with program LAMP, generating approximately 20 functional words/phrases (e.g., ramen, yes, no, I want coke). Anthony previously received speech therapy services; however, it has been two years since he last participated in intervention. There are no concerns with fluency, voice, or feeding at this time. Anthony's primary language is English, therefore, this evaluation was conducted in English. Subsequently, Anthony has a diagnosis of Autism Spectrum Disorder (ASD) Level 3 since age 3. He was born at 39-weeks via vaginal delivery, weighing 7lbs 2oz, and required six separate blood transfusions in the neonatal period. No other hospitalizations reported. His vision and hearing are within normal limits, with no current concerns or impairments reported. There is no history of surgical procedures.

The parents/caregivers will be responsible for incorporating his Home Management Intervention Program at home. The frequency of therapy sessions will be 2x/week for 30-45 minutes.

Information was gathered via parent report, skilled clinical observation, and standardized assessment. During the evaluation, Anthony presented as distractible and active, requiring numerous prompts to maintain cooperation and demonstrating moderate delays in his response rate. He appeared socially withdrawn with poor awareness of others and his environment, which may have contributed to the questionable reliability of some assessment scores. A comprehensive review of findings indicates profound and significant deficits across multiple functional domains, including fine motor coordination, sensory processing and regulation, activities of daily living (ADLs), and social-emotional skills. These deficits pervasively impact his ability to safely and independently participate in age-appropriate occupations at home, in school, and within the community. Formal assessment of motor skills using the Bruininks-Oseretsky Test of Motor Proficiency, 2nd Edition (BOT-2), revealed profound deficits in fine motor control. Anthony achieved a Fine Manual Control composite standard score of 20, which is more than -3.0 standard deviations below the mean and falls at the less than 1st percentile for his age. Subtest analysis further highlights the severity of this delay, with both Fine Motor Precision and Fine Motor Integration yielding age equivalencies of less than 4 years. He is unable to produce legible handwriting at a functional speed, manipulate classroom tools effectively, or participate in detailed science labs. Assessment of Goals/Response to Treatment: Anthony presented to the session alert, oriented, and cooperative, demo'ing excellent engagement and motivation throughout all therapeutic activities. Skilled OT services were to address ADLs, FM coordination, cognitive integration, and psychosocial skills through structured and task-based interventions. Significant progress was noted in UBD, as Anthony I manipulated all 5 fasteners on a front-opening shirt to don and doff the garment in 1/1 opportunity, demo'ing improved B integration, FM coordination, and sequencing. LBD was also addressed, c Anthony I donning and doffing shorts in 2/2 trials, indicating improved motor planning and I c lower body ADLs. To target oral hygiene and sustained attention, Anthony participated in a simulated tooth-brushing task, achieving ~90% accuracy in cleaning a 3-D model. He req. mod verbal cues to sustain engagement for the full 2-minute duration, indicating continued need for support c attention. Cognitive and FM skills were further addressed through completion of two 24-piece puzzles. Anthony req. min cues for problem-solving and mod physical A, targeting B integration and crossing midline. Overall, Anthony demos significant progress in ADL I, particularly in dressing tasks, c continued need for intervention targeting sustained attention and consistency across functional activities. He continues to benefit from skilled OT services to further improve I and participation in daily routines.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you. Please feel free to call our offices if you have any questions. 03-12-2026 12:59 PM	Rachael Haduck	Hi! I wanted to reach out regarding Anthony’s attendance and make-up sessions. At this time, Anthony has 12 make-up sessions that need to be completed within his current authorization period. Many of these accumulated due to illness and doctor’s appointments, which is completely understandable. However, it is very important that make-up sessions are completed when they are offered. Insurance and New Beginnings typically expect families to maintain about a 90% attendance rate in order to avoid any potential reduction in approved therapy sessions. I also want to note that the number of make-up sessions Anthony needs to complete for OT may be different from speech, as each discipline tracks attendance separately. I completely understand that the car situation can make scheduling very difficult right now :( , but if possible, please try your best to attend our schedule make-up sessions when they are available. I want to make it as easy as possible with the availability I have, as well as Lauryn's being more flexible than me. :) Please let me know if you have any questions or concerns! I'm here to help in any way I can!!
