Feeding Background History: Annalia is a 7-year, 6-month, 9-day-old female referred for a feeding evaluation due to long-standing concerns regarding feeding difficulties, including coughing with liquids, food jagging, limited dietary variety, and concerns regarding low weight relative to height. Her mother, Ms. Moreno, and maternal grandmother served as the primary informants and provided developmental and medical history. Caregivers report that feeding difficulties began in infancy. Annalia did not successfully latch during breastfeeding and was transitioned to bottle feeding. Upon introduction of purees and solid foods, Annalia demonstrated aversion and gagging, with limited interest in eating. These feeding challenges have persisted across development. A primary caregiver concern is chronic coughing with liquids, which has been present for an extended period and continues to impact feeding safety and efficiency. Additionally, Annalia demonstrates food jagging, characterized by repeated consumption of a narrow range of preferred foods followed by elimination of those foods from her diet. She exhibits strong texture preferences, favoring crispy foods, and avoids harder or dense textures such as crusts. Sensory sensitivities are also reported, including aversion to loud noises, which may impact overall regulation during mealtimes. Despite these challenges, caregivers report that Annalia is sometimes willing to try new foods, and family goals include expanding her food inventory and improving nutritional intake. Annalia's feeding and swallowing abilities were assessed via a bedside swallow evaluation and structured mealtime observation to evaluate his current functional feeding skills and identify contributing factors impacting feeding performance.
Feeding Medical History: Annalia was delivered via emergency Cesarean section due to fetal heart rate deceleration; maternal heart rate changes were also reported. No neonatal complications requiring prolonged medical intervention were reported. Annalia received a diagnosis of Autism Spectrum Disorder in 2024. Her pediatrician has expressed concern regarding underweight status relative to height. Caregivers report intermittent constipation, which is currently managed with increased water intake. No history of reflux, vomiting, or chronic respiratory illness was reported. No sleep concerns were endorsed.

To stay ahead of this change and help prevent any interruptions in your child’s therapy, we would like to know how you would like to proceed. The options to proceed would be to remain with Dr. Patel and switch to a different insurance plan, or remain with Superior Health and transition to a different pediatrician. If you choose to stay with Dr. Patel and switch insurance plans, we will need a new referral for all services under the new insurance, as well as a copy of your new insurance ID card so we can verify benefits. If you choose to switch pediatricians and remain with Superior Health, we will need a new referral for all services from the new primary care provider (PCP). Please contact the front office as soon as possible so we can help coordinate this change and ensure there are no gaps in your child’s therapy services. Thank you for allowing us to serve your child!

I also have 1pm open on Wednesday - let me know what time works best. This would be for next week and moving forward. Therapy will be once a week for 30 minutes.
