Speech Background History: Bodi, a 3-year, 10-month-old male, was referred for a comprehensive speech and language evaluation to determine his eligibility for speech and language services and to assess his current level of functioning due to concerns of speech and language delays. The evaluation was completed through a combination of standardized testing, parent interview, and skilled clinical observation. Bodi’s mother provided the developmental and medical history. She reported that her pregnancy was considered high risk and that she was induced at 37 weeks due to preeclampsia. Bodi was born with an extra thumb, which was surgically removed at approximately 18 months of age. He was diagnosed with Autism Spectrum Disorder (ASD) in July 2023 and with epilepsy, for which he currently takes Keppra. Bodi has experienced six ear infections and has had two sets of ear tubes placed. His most recent hearing evaluation, conducted in November 2023, revealed normal hearing. His tonsils were removed due to breathing concerns, and he has passed his vision screening. Bodi’s mother expressed ongoing concerns regarding feeding behaviors, noting that he primarily eats with his hands, refuses to use utensils, and is selective with his foods, preferring crunchy textures such as chicken and snacks. Bodi demonstrated MIN-NO imitation, requiring MAX multi-sensory support, benefitting most from HOH support. Assessment of Goals/Response to Treatment: Bodi presented as alert, oriented, and cooperative, demo'ing an eagerness to engage in therapeutic activities. Skilled intervention focused on improving motor, psychosocial, and cognitive integration skills through structured tasks. During tabletop activities, including puzzles and peg manipulation, Bodi req. mod A and verbal cues to facilitate task initiation and promote sustained attention, VMI, and problem-solving. Progress was noted in grasp development and visual processing as Bodi participated in these activities. Regarding sensory regulation goals, Bodi demo'ed emerging tolerance for sensorimotor input, engaging in swinging for a 30-second interval before eloping from the activity. Furthermore, significant progress was made in safety awareness and self-regulation, as Bodi exhibited zero instances of non-functional mouthing of objects during the session. For functional mobility, Bodi continues to req. min A to safely navigate transitions throughout the building and in the parking lot, indicating a need for ongoing support to ensure environmental safety.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. Our telehealth sessions are conducted securely via Zoom, allowing your child to receive high-quality, personalized OT from the comfort of your home. Once your child's in-person spot becomes available, you will have the option to come into the clinic for therapy or, if you prefer, continue with telehealth if your child is a good candidate. The address is 1510 N Zaragoza Rd, Suite B1, El Paso, TX 79936. It is very close to the current building, and I’m happy to send a photo showing its location in relation to where we are now if that would be helpful, just let me know! I’ve already checked out the space, and it has a lot of great materials and activities that I’ll be able to use during our sessions. My first official day at the new location will be February 2nd, so please still come to the building you have been until the end of the week. So because the reevaluation has not happened this will effect his OT visits starting next week. First we need the referral, then the reeval will happen, and then once everything is signed and approved we can start visits back again. I don't know if that makes any sense, if not I can go over it in person tomorrow
