Speech Background History: Joshua is a 6-year 7-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation. Joshua’s mother served as the informant for the evaluation. Joshua’s mother reported that he was diagnosed with autism, ADHD, anxiety, and a sleep disorder when he was approximately 3 years old. Joshua’s mother reported concerns with his language skills stating that he is non-verbal, however, uses a combination of vocalizations, gestures, and an AAC device with the TD snap application to facilitate communication. Assessment of Goals/Response to Treatment: In 6 months time, Joshua will imitate gestures, signs and/or words with 40% accuracy in order to increase imitation and word and/or sign use. - During semi-structured activities, Joshua imitated most clinician models (verbal +sign) for a a variety of pragmatic functions and carried over with ~20% accuracy with verbal approximation. In 6 months, Joshua will use 1–2 words using total communication (verbal speech, signs, or AAC) for a variety of pragmatic functions (greetings, commenting, requesting, protesting) with 40% accuracy in order to facilitate functional communication skills with family, caregivers, and peers during day to day tasks. - During semi-structured tasks, clinician modeled many 1-2 verbal + sign combinations for a variety of pragmatic functions. Joshua imitated in ~50% of opportunities. He IND carried over models in ~20% of opportunities given MOD multi-sensory input. Occupational Background History: Joshua is a 6.7 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Racheal, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression and adverse behaviors. Joshua was able to sit and engage in standardized testing although requires max redirection to task, with avoidant behaviors and severe self regulation deficits when transitioning away from preferred task. Child attempted to flee, fell to floor, attempted to hit wall or kick doors, requiring max A in order to calm. Mother reports frequent episodes lasting extended time periods.

Subjective: Patient was eager to participate,Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Patient presented with a cooperative and eager disposition for today's skilled intervention. Progress was noted in psychosocial regulation, as the patient successfully transitioned between 3 therapeutic activities with good responses, requiring only 1-2 verbal prompts when paired with a visual timer to facilitate task initiation and termination. To address functional goals for upper body dressing, the patient engaged in a gross motor task and demonstrated improved coordination and bilateral integration by successfully completing 10 repetitions of a reaching and passing activity across midline. Intervention then focused on activities of daily living, where the patient required maximum physical and verbal guidance to imitate hand and wrist mobility movements for hygiene. With hand-over-hand assistance, the patient was able to approximate the correct motor pattern through all four quadrants for 2 out of 5 trials, indicating an ongoing need to address motor planning and imitation. The patient demonstrated significant gains in sensory processing by fully participating in a sensorimotor play activity, utilizing a pincer grasp to retrieve 15 small items from a slime texture without any signs of tactile aversion. Following retrieval, the patient successfully sorted all items by color with 100% accuracy, demonstrating intact visual perception and sustained attention for the task.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. 11-11-2025 11:21 AM	Rachael Haduck	he did well! what I explained to aunt is that it was an informal session- this allows for us to build rapport, get to know each other, and understand what level of support he needs and what he is capable of. On Thursday, I am going to introduce AAC (Augmentative and Alternative Communication) which is text to speech. This allows for him to communicate his needs and wants when he is not able to verbally. if he is a good candidate, we can see about getting him a device in the future. :) If you want to look it up, you can view program likes TouchChat, NovaChat, TDSnap, etc. to have an understanding of what it is.

I spoke to Kassandra and she is able to offer 8:00am, then speech would be at 9:30am due to me having a reoccurring 9:00am appointment.
