Occupational Background History: Kaileb is a 9.3 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per mother's report Kaileb has difficulty with sequencing steps, fine motor skills, cognitive delays leading to requiring mod to max A with ADL and IADLs. During the evaluation Kaileb required redirection to task, repetition of steps, and HOHA for fine motor skill tasks.

Occupational Background Summary: Kaileb demonstrates difficulty with fine motor skills due to limitations with object manipulation, manual dexterity and manual control evident by premature grasp patterns, spillage and difficulty with small objects limiting fine motor precision. Kaileb demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: ensorimotor, play-based approach w/ positive reinforcement utilized to address deficits in motor planning, bilateral coordination, FM control, sensory processing, and cognitive integration. During buttoning task, pt required MAX PA (HOH) to manipulate 3 one-inch buttons. Pt demo decreased pincer grasp strength, poor finger isolation, and limited bilateral coordination, requiring therapist facilitation for stabilizing fabric w/ one hand while manipulating button w/ contralateral hand. Pt also required assist for alignment and force grading, indicating deficits in praxis and in-hand manipulation skills. During UB dressing task (donning pullover shirt), pt required MAX PA for garment orientation, sequencing, and execution. Pt demonstrated difficulty initiating task, orienting clothing correctly, and coordinating head and UE placement, indicating impairments in body awareness, motor planning, and sequencing. Pt required step-by-step facilitation and continuous VC/PA for successful completion. Progress observed in toileting hygiene; pt followed 3-step visual sequence for wiping w/ mod VC in 1/1 trial. Pt demonstrated improved ability to attend to and interpret visual supports, sequence steps, and complete routine, though continued cueing required for thoroughness and task completion. This reflects emerging independence in familiar ADL w/ structured supports. During oral hygiene activity, pt tolerated toothbrushing for ~30 sec w/ MAX PA before exhibiting s/s sensory discomfort (avoidance, facial grimacing). Pt required full assist for utensil control and sequencing. Limited tolerance indicates ongoing oral sensory defensiveness and decreased ability to sustain participation toward 2-min goal. Across tasks, pt required frequent redirection and multimodal cueing (VC, tactile cues, HOH) to sustain attn and complete activities. Performance impacted by decreased motor planning, reduced bilateral integration, FM weakness, and sensory modulation difficulties. Pt demonstrates emerging progress in ADL participation, particularly in toileting hygiene w/ use of visual supports. However, pt continues to present w/ significant deficits in praxis, bilateral coordination, FM control, sensory processing, and sustained attn, resulting in dependence on MAX A for most self-care tasks. Oral sensory defensiveness and decreased grasp strength continue to limit independence.
