Speech Background History: Richard is a 5 year, 8 month old male who was seen on the above date for an initial evaluation per referral of his PCP to determine his current level of functioning and whether skilled speech-language intervention is warranted at this time. He was accompanied by his mother, who provided pertinent verbal and written information referenced in this report. English is the primary language spoken in the home and academic settings. Richard began receiving speech, occupational, and physical therapy services in 2023. Richard was diagnosed with Autism Spectrum Disorder (Level 3) in 2024 and also carries a diagnosis of ADHD. Per mother report, Richard has been introduced to AAC and currently uses his personal iPad with TD Snap for communication. She also shared that Richard does not show interest in toys; however, he enjoys objects that produce music and lights.

Assessment of Goals/Response to Treatment: Richard presented to the session alert and oriented; however, he demo'ed decreased sustained attention, req. frequent verbal cues to return to therapist-directed tasks. Skilled OT services were provided to address ADLs, sensory regulation, VMI, and psychosocial skills through a structured work-reward sequence. To address safety awareness and sensory regulation, Richard req. mod verbal and tactile cues throughout the session to refrain from mouthing non-edible items, indicating continued deficits in impulse control and self-regulation. Progress was noted in self-care, as Richard donned and doffed a pullover shirt req. only min verbal cues for sequencing in 1/1 opportunity, demo'ing improved I c UBD. During a functional feeding task (pudding), Richard demo'ed improved utensil management and VMI, I completing 3/5 scoops. He req. mod physical A for the remaining 2/5 scoops to support appropriate grasp and motor execution. The session incorporated a work-reward sequence utilizing a preferred activity (bubbles), which effectively increased motivation and facilitated participation in all therapist-directed tasks. Overall, Richard demos measurable progress in UBD and feeding skills, c continued need for intervention targeting sustained attention, sensory regulation, and safety awareness. He continues to benefit from skilled OT services to improve I c ADLs and increase functional participation.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. The address is 1510 N Zaragoza Rd, Suite B1, El Paso, TX 79936. It is very close to the current building, and I’m happy to send a photo showing its location in relation to where we are now if that would be helpful, just let me know! I’ve already checked out the space, and it has a lot of great materials and activities that I’ll be able to use during our sessions. My first official day at the new location will be February 2nd, so please still come to the building you have been until the end of the week. 03-03-2026 04:08 PM	Rachael Haduck	hi! yes i'm okay!! thank you for checking in! he bit my finger but I'm not sure what was unfavorable to him in the moment. :( he did really well in the beginning, however, we are taking a step away from bubbles (we used to do them for about 5-minutes per session in the beginning so I think him knowing there are bubbles in my cabinet but hidden out of sight, he gets upset knowing we aren't doing them he will request for them repeatedly on device. so that could be why/on top of not being able to express his frustration :(

I'll have his dad talk with him as well. I'll take away the bubbles option on the tablet this way he can't be tempted to choose that. He's really good with repetition but then he gets use to it and doesn't want change so it's a tricky one for sure. Once again I'm so sorry, he bit me once out of fear at the doctor so I know that it hurts like hell! Please lmk if there's anything I can do to help or if he's ever frustrated like that again I can come in and help calm him. I'm here to help and open to all suggestions!

He was constipated and I thought that might've just been it because he did use the bathroom really bad yesterday at school, but they called me to pick him up today and now he's running a fever so I'm thinking he might have a stomach bug. They said it's going around at their school. So I think I'll just keep him home and maybe we can schedule a make up session next week sometime when he's better.

The front office mentioned that they are currently pending the doctor’s signature. If you would like, you may be able to call the doctor’s office to help speed up the process. I just wanted to give you a heads-up because if I do not receive confirmation of the new authorization before his next visit, I unfortunately will not be able to see him for OT.
