Child currently demonstrates attention span approximately 10 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection.

Occupational Progress to Date: Ruben is a 7-year, 3-month-old male who presents for this occupational therapy evaluation to establish his current level of function. According to parent report, he has an unremarkable medical and developmental history. During the evaluation, Ruben was talkative and active, requiring numerous prompts to maintain cooperation and attention to task. His distractibility and moderately delayed response rate suggest that standardized scores should be interpreted as a baseline measure of performance, which is consistent with parental concerns regarding attention and self-control. Objective assessment using the Bruininks-Oseretsky Test of Motor Proficiency, 2nd Edition (BOT-2) revealed significant deficits in motor skills. Ruben achieved a scale score of 7 in Fine Motor Precision, placing his performance -1.0 standard deviation below the mean for his chronological age. This score equates to an age equivalency of 6 years and 3 months, representing a significant delay of 12 months. His overall Fine Manual Control composite standard score of 89 places him at the 23rd percentile, which is below the age-expected 50th percentile. Subjective: Patient was distractible and inattentive,

Assessment of Goals/Response to Treatment: The patient presented as distractible and inattentive during today's session, which impacted engagement in therapeutic activities. Skilled occupational therapy intervention focused on enhancing psychosocial, cognitive, and visual-motor skills through structured cooperative play activities. Progress was noted toward maintaining appropriate personal space; during a group activity, the patient successfully maintained an arm's length distance for up to 1 minute with moderate verbal and visual cues on 2 out of 5 opportunities before leaning on peers, indicating an emerging awareness but continued need for support. Challenges with self-regulation were observed during transitions. When provided with a visual schedule and verbal warnings to transition from a preferred to a non-preferred task, the patient required 4-5 verbal prompts and cried for approximately 2 minutes, demonstrating continued difficulty with this skill. Frustration tolerance was also addressed when the patient's building block structure fell. Initially, the patient became dysregulated, stating, "I can't do this," and attempted to leave the task. With moderate verbal cues, the patient was guided to use a deep breathing strategy and was able to return to the task after 4 minutes on 1 of 2 opportunities. Overall, the patient requires ongoing moderate assistance to manage sensory-based behaviors and improve self-regulation to facilitate greater participation in functional activities.

Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
