03-13-2026	Kayla Merrick	Date: 3/13/2026 - 3:45 PM \| Occurrence: 2nd Incident To address LTGs for Social Reciprocity and Turn-Taking, the clinician established a boundary by moving the patient's personal toy to a "spectator" role during a structured pragmatic and turn taking activity. Per EBP, this reduces "Object Engagement" (an avoidance strategy) and supports Joint Attention necessary for skilled pragmatic intervention. Upon setting the boundary, the patient became irate, exhibiting high-volume screaming, task refusal, and "freezing." She rejected all clinical co-regulation support. The patient went to the toy room. The clinician informed the parent that this area is off-limits for safety; however, the parent was noncompliant with this directive, stating she could not remove the child because she was upset and was unable to convince her to get up. The patient eventually ran into the lobby screaming, disrupting the clinic waiting area. The parent was informed that the patient's inability to accept boundaries is a primary barrier to progress. The parent will attend future sessions to support facility safety rules and environmental boundaries. 11-21-2025	Kayla Merrick	Date of Incident: 11/21/2025 Time of Incident: 3:45 PM - 4:30 PM Reporting Clinician: Kayla Merrick Patient Information: Evelyn Chain \| Patient Initials \| EC \| \| Age \| 8 years \| \| Gender \| Female \| Incident Description: The incident was initiated with the Evelyn, an 8-year-old female, who became resistant and highly uncooperative after being denied immediate access to a specific, highly preferred toy right away not designated for the scheduled activity at this time. The clinician ensured the client that they would play the desired game later in the session. The client still exhibited marked behavioral dysregulation, escalating to yelling, crying and demonstrating disrespectful verbalizations toward the speech-language pathologist assistant (SLP-A). All attempts at SLP-A led redirection and de-escalation were refused. Intervention and Outcome: Due to the persistent refusal to participate and the high level of vocal distress, the client's mother was requested to enter the therapy space. The mother engaged the client in negotiation regarding participation. Following this intervention, the client de-escalated and agreed to participate in the planned structured therapy activities for the remainder of the session. Follow-up/Recommendations: Future sessions should include proactive planning for transitions and item refusal. It is recommended to collaborate with the parent on a consistent behavioral strategy to address demand refusal and emotional regulation when the clinician makes decision regarding session structure.

So when you have an opportunity can you answer these questions for me: 1.) Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.) 11) Dominate Language ? Then Evelyn born and she stayed at NICU for 8 days (subgaleal hematoma) 2) No. She is diagnosed with Autism, ADHD and Anxiety. 3) Normal that I know. 4) She doesn't have problems to sleep. I notice that when she has a really active day she wakes up a little bit (like half awake half sleep) several times. She sleeps formally from 8 pm to 5:40 am. She snores sometimes but not always. Breathing problems when she is sick and has mucus. Pediatrician said she has big tonsils. We opt out for tonsil surgery. 5) Moxazyd 40 mg per day, fluoxetine 30 mg per day, focus factor 2 chewables per day, smarty pants 4 gummies per day, chillax 3 per day, fungies 2 per day 6) 3rd grade - regular education classroom with special education, co-teaching 7) None 9) Speech concerns started when she had 2.5 to 3 years of age 10) She had OT for 3 years, she finished her therapy. She had 4 years aprox of ST, she didn't finishes her therapy in the previous place, they closed (OT for TOTS)

Her authorization date ends on 4/24/2025 and her evaluation was completed on 10/11/2025. Insurance allows for evaluations to be completed 120 days from the evaluation date, which would begin on February 8th. I am due with my baby in March and will not be able to come in to evaluate in the months of March and April, so I am scheduling evaluations allowed to be completed in February. We can wait until the end of February if you'd feel more comfortable. Please let me know if you want to keep February 10th or push it further. Thank you

Evelyn is currently exhausted and truly needs this spring intersession to rest and recover. Additionally, please note that four sessions per week is not feasible for us. We recognize that the short notice for the March 6th cancellation was an administrative issue, it is in no way a reflection of you, and it will not be recorded as a missed session on your part. We greatly value your consistent attendance and the respect you show in keeping timely appointments. To ensure we are on the same page, we have her scheduled for the following 45-minute sessions through March and April: - Monday, 3/9: 1:30 PM - Tuesday, 3/10: 9:45 AM - Friday, 3/13: 3:45 PM - Friday, 3/20: 3:45 PM - Monday, 3/23: 1:30 PM - Friday, 3/27: 3:45 PM Following these dates, we will move back to normal to a weekly recurring schedule of Fridays at 3:45 PM throughout April starting 4/3.
