Speech Background History: Romero Sirmans is a 6-year, 4-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to concerns regarding limited language skills. Romero’s mother served as the primary informant for the evaluation. The evaluation was conducted in English, as this is the primary language spoken in the home. Romero’s mother reported that he was born full term; however, labor was induced due to gestational diabetes. No complications were reported following delivery, and Romero did not require a NICU stay. According to his mother, Romero’s early developmental milestones were within normal limits. However, around the age of two, she observed a significant regression in speech and language skills. She stated that Romero stopped talking almost completely and used only a few words. Following these concerns, Romero was diagnosed with Autism Spectrum Disorder (ASD). Romero received early intervention services through Early Childhood Intervention (ECI) but recently discontinued due to scheduling conflicts. His mother expressed ongoing concern about his expressive language and comprehension abilities. She reported that Romero has difficulty answering questions using full sentences and often struggles to express complete thoughts during conversation.

Speech Assessment Summary: Romero is a 6-year, 9-month old male who is demonstrating a Mild-Moderate Mixed Receptive and Expressive Language Disorder (F80.2) secondary to Autism Spectrum Disorder (F84.0). These deficits impact Romero's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Romero receives services 1x/week for up to 60minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Romero is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 1x/week for 6 months is recommended due to the Romero's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. Medical necessity for skilled speech and language therapy services is indicated as Romero is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Assessment of Goals/Response to Treatment: Client was an active and willing participant in today's session, completing all structured therapeutic tasks. Skilled intervention focused on improving cognitive, psychosocial, and motor skill deficits through participation in goal-directed activities. To address deficits in activity initiation, the client was presented with a sequence of preferred and non-preferred tasks. He initiated 2 of 4 non-preferred tasks, including a tabletop pegboard activity, but required significant verbal cueing (8-10 cues) to transition and begin, demonstrating emerging progress toward the goal of increased independence. During a cooperative board game, intervention targeted sustained attention and self-regulation. The client was able to maintain an optimal state of alertness and attend to the game for approximately 90 seconds with moderate verbal and tactile cues to redirect his focus and remain on task. This same activity was utilized to address social participation goals; the client engaged in turn-taking with the therapist for 3 minutes, requiring 7 verbal prompts to await his turn and share game pieces, showing progress in social engagement. Fine motor and cognitive sequencing skills were addressed using a shoelacing board. With moderate assistance, including visual cues and hand-over-hand guidance for one step, the client successfully sequenced 2 of 4 visual steps for the lacing task, indicating developing in-hand manipulation and problem-solving skills. Continued skilled occupational therapy is medically necessary to improve functional independence across all goal areas.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. We understand that timely care is essential. To help avoid further delay, we want to offer you early access to therapy via telehealth services. This option will get your child's therapy started now, while you wait for in-person therapy. Our telehealth sessions are conducted securely via Zoom, allowing your child to receive high-quality, personalized OT from the comfort of your home. Once your child's in-person spot becomes available, you will have the option to come into the clinic for therapy or, if you prefer, continue with telehealth if your child is a good candidate. Please let us know if you are interested in starting early or just waiting in line for in-clinic therapy. Thank you!

Can you please reach out to his doctor and ask them to send the referral to Tricare? Let me know if you have any questions.

I'm confused because I just received a call last week saying he can return to OT. He missed two weeks in awaiting. I'm confused to have received a call if this was indeed not handled.

His current OT authorization is good until 4/24/26, but in order for him to continue to have authorization after 4/24/26 we will need a new referral. The reason I am letting you now, is so you can have time to get the referral before we lose authorization. I hope this makes sense.
