Patient presents with weakness, decreased mobility, decreased balance, lacks coordination, atypical gait, decreased activity tolerance, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 15 month old male who scored at 12 month level in Body Control on the PDMS-3 assessment indicating a below average status simple and complex skills; thus moderately impacting development. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers.
Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved.
Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the family as part of a home program (if applicable) at the end of every session. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.
Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We would like to inform all patients with Blue Cross Blue Shield (BCBS) insurance that insurance benefits reset at the beginning of the new year. This means that all copays, coinsurance amounts, deductibles, and out-of-pocket maximums will reset.
As a result, payments for visits will be charged at the full allowed amount until your deductible is met. Depending on your specific BCBS plan, you may or may not have a deductible.
Below are the allowed amounts that will apply until your deductible is met:

Re-evaluation: $234.19
Feeding Therapy (FD):
