Speech Assessment Summary: Alex is a 5-year, 3-month old male who is demonstrating a Severe Mixed Receptive and Expressive Language Disorder (F80.2) secondary to Autism Spectrum Disorder (F84.0). These deficits impact Alex's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Alex receives services 2x/week for up to 60-minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Alex is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Alex's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Alex is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Alex will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Any changes to this treatment recommendation will be reported to the referring physician in the next progress summary or evaluation report whichever comes due first as per state/federal regulations. LANGUAGE: The Preschool Language Scales Fifth Edition (PLS-5) is an individually administered instrument used to assess a child's receptive and expressive language skills from birth to age 7:11. The assessment identifies receptive and expressive language skills in the areas of attention, gesture, play, vocal development, basic concepts, language structure, integrative language, and literacy development. The PLS5 consists of the Auditory Comprehension standardized scale and the Expressive Communication standardized scale to obtain a Total Language Score and determine if the child is presenting with a language delay or disorder. Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: Alexander was engaged and cooperative throughout the occupational therapy session, completing all structured therapeutic activities presented. Notable improvement was observed in self-regulation, as he independently maintained an appropriate level of alertness and arousal to complete a one-minute tabletop task without the need for verbal or physical cues, marking significant progress from previously requiring maximal support. In targeting sensory integration, an auditory stimulus resembling a child crying was introduced. With moderate verbal and gestural prompting, Alexander successfully implemented a coping strategy by covering his ears and practicing deep breathing. This allowed him to remain in the therapy setting for the full one-minute duration during the trial without engaging in maladaptive behaviors such as yelling or hitting, reflecting meaningful progress in tolerating challenging sensory input. Fine motor and visual-motor skills were addressed through a simulated feeding activity. Alexander used a spoon with moderate spillage and attempted to use a fork, requiring maximal hand-over-hand assistance to pierce food items. These observations indicate that grasp patterns and in-hand manipulation skills related to utensil use continue to require development. He also demonstrated strong cognitive integration abilities, including sequencing and problem-solving, during a multi-step sensorimotor play task. Overall, Alexander is making positive gains in self-regulation and coping strategies. Continued skilled intervention is recommended to further improve independence with feeding tasks and reduce the level of support needed to initiate and apply sensory coping strategies.

To stay ahead of this change and help prevent any interruptions in your child’s therapy, we would like to know how you would like to proceed. The options to proceed would be to remain with Dr. Patel and switch to a different insurance plan, or remain with Superior Health and transition to a different pediatrician. If you choose to stay with Dr. Patel and switch insurance plans, we will need a new referral for all services under the new insurance, as well as a copy of your new insurance ID card so we can verify benefits. If you choose to switch pediatricians and remain with Superior Health, we will need a new referral for all services from the new primary care provider (PCP). He is started to just say more and beginning to try and say some words but not a clear things. He initially was nonverbal for a long time but as soon as he started school he started grasping on the concept of pronouncing things. Currently, Alexander is scheduled with me on Thursdays at 4 PM; however, there is no availability at or after 4 PM with any therapist, as all after-school time slots are fully booked. Because of this, I wanted to see if you might be available for a 2 PM appointment on either Wednesdays or Fridays. This change would be permanent. Additionally, there will be a transition to another therapist during my leave. The therapist who takes over his care at that time will remain his permanent therapist moving forward, along with the new 2 PM schedule. I want to make sure Alexander continues receiving consistent care and is able to maintain his therapy schedule of 2x per week. Please let me know what works best for you. Thank you so much for your understanding and flexibility.
