Speech Assessment Summary: Benjamin is a 3 year, 6 month old male who was re-evaluated today to determine continued need of speech therapy services. Benjamin was accompanied by his mother during the evaluation. Mother reported that Benjamin received an autism diagnosis on November 2025. He is not currently taking medications based on parent report. The Preschool Language Scales (PLS-5) was used to assess his current language skills. The evaluation was completed in English and Spanish, as it was noted that he responds to both languages and is beginnings to express language mainly in English. The following were his scores: Auditory Comprehension - SS 61 (Raw Score 22), Percentile Rank 1 Expressive Communication - SS 50 (Raw Score 26), Percentile Rank 1 Total Language - SS 52 (Raw Score 48), Percentile Rank 1 When Benjamin began therapy, he would consistently cry, hit himself, scream, and get upset at anything requested or any attempt with communication. Benjamin can now complete a session without crying and screaming. He is able to stop and gaze to request for help instead of having a meltdown. He is no longer hitting his face or throwing himself on the floor. He can request using total communication. He is responding to mitigated phrases to request with maximum cueing and modeling, along with total communication. He likes his routine and has difficulty when it changes. Assessment of Goals/Response to Treatment: The patient presented as alert, cooperative, and demonstrated a high level of engagement throughout the session. Skilled intervention focused on advancing fine motor, visual-motor, cognitive, and psychosocial skills to support progress toward self-care independence. During a structured fine motor task involving pegs and a pegboard, the patient worked on sorting by color, which addressed eye-hand coordination, matching to sample, and dexterity. The patient required tactile cues to facilitate a functional grasp and squeeze for peg manipulation. To address the self-care goal related to hygiene, the patient engaged with a mouth model and a video prompt to sequence and imitate hand and wrist mobility movements necessary for cleaning. This activity specifically targeted motor planning and the ability to cross midline to reach all four quadrants. Progress toward the goal of sequencing 3 out of 5 visual steps for a hygiene routine is noted at 65%; while the patient continues to require total assistance for initiation and sequencing accuracy, his ability to imitate demonstrated movements is an emerging strength. Positive reinforcement was consistently provided to enhance transitions between tasks, resulting in good follow-through and promoting the development of effective coping and activity initiation skills.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache.
