Speech Background History: Shepherd Garrett is a 4-year, 6-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to concerns regarding limited language skills. Shepherd’s mother served as the primary informant for the evaluation. The evaluation was conducted in English, which is the primary language spoken in the home. Per parent report, Shepherd was born at 38 weeks gestation due to maternal preeclampsia. There was no NICU stay required. At one year of age, Shepherd was hospitalized due to a respiratory illness and a heart murmur; however, his mother reported that the heart murmur has since closed. Developmental milestones were reportedly within normal limits. Shepherd has a history of recurrent ear infections and currently has pressure equalization (PE) tubes placed in both ears. His mother shared that one of the PE tubes recently shifted, contributing to episodes of muffled hearing. The pediatrician noted no further action was required; however, the evaluating clinician recommended a referral to an ENT specialist for further assessment of Shepherd’s hearing and PE tube function. Speech Assessment Summary: Shepherd is a 4-year, 11-month-old male who was re-evaluated in August 2025 to determine continued eligibility for speech-language therapy services due to concerns regarding limited language skills and reduced speech intelligibility. English is Shepherd’s primary and dominant language; therefore, all assessments were administered in English. Shepherd presents as neurotypical and participated appropriately in structured and play-based assessment tasks. Standardized test results should be interpreted alongside clinical observations and caregiver report to best reflect Shepherd’s functional communication abilities across environments. The Clinical Assessment of Articulation and Phonology–Second Edition (CAAP-2) was administered to evaluate Shepherd’s speech sound production and phonological development. Shepherd obtained a Consonant Inventory Score of 23, corresponding to a standard score of 79 and an age equivalent of 2 years, 8 months. Shepherd demonstrates a MILD ARTICULATION/PHONOLOGICAL DISORDER that continues to impact speech intelligibility in connected speech.This score falls approximately 1.5 standard deviations below the mean when compared to age-matched peers, indicating a mild articulation/phonological disorder. Observed speech sound errors included gliding, errors with /r/ clusters, vowelization, and substitution of /d/ for voiced and voiceless /th/. Despite this progress, Shepherd continues to present with reduced articulatory precision, particularly in connected speech, and benefits from cues to over-pronounce and increase speech clarity, as mumbling continues to impact intelligibility in functional contexts. Based on formal assessment results, parent report, and clinical observations, continued speech-language therapy is medically necessary. Shepherd demonstrates a MILD ARTICULATION/PHONOLOGICAL DISORDER that continues to impact speech intelligibility in connected speech, with AGE-APPROPRIATE CORE LANGUAGE SKILLS and EMERGING PRAGMATIC LANGUAGE DIFFICULTIES affecting topic maintenance and conversational reciprocity. Although Shepherd has demonstrated progress, his speech intelligibility remains below age expectations, particularly in connected speech and conversational contexts. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. I called Tricare to verify that the cap has been met and they informed us it has been met, so you will no longer get charged for appointments! Regarding the previous appointments that you were charged for the Tricare representative also stated that Tricare will reimburse you as well. I would advise you to call them to get that information from them, as the representative on the phone stated their is a form for you to fill out. I also wanted to advise you that at the start of next year, 01/01/2026, The catastrophic cap does restart so we will start charging the co-pay after that date.
