His sustained attention to non-preferred task was about 30 seconds. During play-based activities (i.e., farm animals), the patient demonstrated joint attention and imitation skills through play. Patient presented with difficulty in attending to task and following simple directions (i.e., clean up). Patient played with items presented to him demonstrating solitary play skills.

Mateo’s mother served as the primary informant for this reevaluation. Mother continues to report concerns regarding Mateo’s limited expressive vocabulary and difficulty communicating his wants and needs using spoken language. Since the previous evaluation, Mateo has demonstrated some progress in vocabulary development. Mother reports that Mateo now produces approximately 20 single words, compared to approximately 15 words at the time of the previous evaluation. Mateo has also begun to attempt two-word combinations, though this skill remains emerging and inconsistent. Parent report indicates that Mateo continues to rely on gestures, pointing, and crying to communicate his wants and needs rather than consistently using spoken language. Mateo continues to be exposed to Spanish and English. Spanish remains Mateo’s dominant language, and therapy services will continue to be provided primarily in Spanish with English support to support communication across environments. Mateo was reevaluated using the Preschool Language Scales Fifth Edition Spanish to assess his receptive and expressive language skills. On the Auditory Comprehension subtest, Mateo received a raw score of 18, corresponding to a standard score of 50, a percentile rank of 1, and an age equivalence of 1 year, 2 months, indicating a PROFOUND RECEPTIVE LANGUAGE DISORDER. On the Expressive Communication subtest, Mateo received a raw score of 22, corresponding to a standard score of 73, a percentile rank of 4, and an age equivalence of 1 year, 7 months, indicating a SEVERE EXPRESSIVE LANGUAGE DISORDER. Mateo’s Total Language Score resulted in a standard score of 59, a percentile rank of 1, and an age equivalence of 1 year, 4 months. These results indicate continued significant receptive and expressive language delays, consistent with a PROFOUND MIXED RECEPTIVE-EXPRESSIVE LANGUAGE DISORDER. Despite these delays, Mateo has demonstrated meaningful progress since the previous evaluation. During therapy sessions, Mateo is increasingly attempting to imitate sounds, words, and actions when modeled by clinicians and caregivers. He is also demonstrating increased engagement with clinicians and peers during play-based activities. Mateo is beginning to follow simple directions, although he continues to require frequent verbal and gestural cues to complete tasks. He is also demonstrating functional and relational play, which supports emerging language development. Receptively, Mateo is beginning to identify familiar objects, though this skill remains inconsistent and emerging. He is also beginning to demonstrate early pretend play skills, which still require adult modeling and support. Expressively, Mateo now produces approximately 20 single words and has begun attempting two-word combinations, although these productions are inconsistent. Mateo also uses gestures such as pointing and showing objects to communicate with others and is beginning to participate in simple turn-taking interactions, though he rarely initiates them independently. Mother reports no recent medical changes, and Mateo is not currently taking any medications. Mateo continues to demonstrate difficulty with transitions and emotional regulation, particularly when activities do not occur in his preferred order or when transitioning away from preferred items. During therapy sessions, Mateo may respond by crying, repeating “no,” or attempting to remove materials when presented with non-preferred tasks. These behaviors appear to be related, in part, to limited communication skills and difficulty expressing his wants and frustrations verbally. At 2 years, 7 months, children typically have a vocabulary of 50 or more words, are beginning to combine two- to three-word phrases, follow simple two-step directions, and engage in pretend play while using language to request, comment, or share attention with others. Mateo currently produces approximately 20 single words and is just beginning to attempt two-word combinations, showing limited ability to follow directions and engage in functional or pretend play with language. This demonstrates that Mateo’s receptive and expressive language skills remain significantly below age expectations, consistent with a profound to severe mixed receptive-expressive language disorder. Overall, based on formal assessment results, parent report, therapy progress, and clinical observations, MATEO CONTINUES TO PRESENT WITH A PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. Although Mateo has demonstrated progress in imitation, engagement, play skills, and vocabulary growth, his language abilities remain significantly below developmental expectations, particularly in receptive language comprehension, expressive vocabulary, and functional communication. These deficits impact his ability to effectively communicate and participate in age-appropriate social interactions and daily routines. Without continued speech-language therapy, Mateo remains at high risk for ongoing communication delays, difficulty understanding directions, and reduced ability to communicate his needs or request help. These limitations may affect his safety, social development, and early learning skills. When you have an opportunity can you answer these questions: 1.) Name and DOB 2.) Any complications with pregnancy or birth? 3.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) when did your concerns begin 9.) In your a few words write you parental concerns with SPEECH and LANGUAGE along with your child’s strengths.

All developmental milestones meet with normal Limits except Speech 4.) No sleeping or breathing issues, yes gets at least 8-10 hours of sleep. No snoring, sleeps with mouth closed. No enlarged tonsils. 5.) Currently not on any medications. 6.) No daycare or preschool. 7.) No vision or hearing issues. 8.) Concern for speech began around a year and a half. 9.) I'm concerned for his speech he is limited to about 10-15 words he doesn't use 2 word sentences, talks with one word only. He is very smart he knows the things he just won't say the words. He uses signs to talk for example when he's hungry he will point to his mouth and then rub his stomach. He understands everything we tell him he just doesn't talk
