Occupational Background History: DX: F91.8 Violetta (Letty) is a 2 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Maria, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per mother's report Letty demonstrates severe temper tantrums which occur frequency and last extended periods of time. Mother reports Letty will also engage in self harming behaviors and attempt to throw objects or hit others as well. Parents report Letty having difficulty with emotional regulation and expression of how she is feeling which leads to tantrums.

Occupational Progress to Date: Per standardized assessment scores Violetta well falls below average in all subsets of the BOT-2 evident by below average age equivalency scores ranging from 12 months to 20 months compared to her chronological age of 2 years and 7 months, placing the child atleast -2 to -3 standard deviations below their peers their chronological age. Although Violetta has had significant improvement evident progress with age equivalency scores going from 9 months to 20 months during this last auth.

Occupational Rehabilitation Potential: Good. with consistent Occupational Therapy and parental support

Occupational Treatment Plan: Violetta is a 2-year and 7-month-old female who continues to demonstrate a medical necessity for skilled occupational therapy to address moderate deficits across multiple functional domains. Her current level of function is characterized by significant delays in fine motor control, cognitive processing, and sensory regulation, which collectively impair her ability to participate in age-appropriate activities of daily living and play. Standardized assessment scores from the BOT-2 quantitatively substantiate these delays, placing her overall functional skills between a 12 to 20-month age equivalency, which is -2 to -3 standard deviations below her chronological age. Functionally, these deficits manifest as immature grasp patterns impacting object manipulation, spillage during self-feeding, and difficulty with tasks requiring manual dexterity. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: The patient presented with a friendly and cooperative demeanor, actively participating in the skilled occupational therapy session. Intervention was aimed at enhancing motor, psychosocial, and cognitive integration skills through a structured functional activity. During the task, which required in-hand manipulation, sequencing, and sorting of objects, the patient was prompted to follow 2-step, related commands to address her goal of improving auditory processing and executive functioning. Across 4 trials, the patient required maximal assistance (MAX A), consisting of repeated verbal cues to maintain sustained attention and consistent hand-over-hand physical guidance to initiate and complete both steps of the directives. With this level of support, the patient was able to successfully complete the command in 1 of 4 opportunities (25%). This performance demonstrates continued significant deficits in auditory processing, motor planning, and sequencing that impact functional independence. While the patient’s willingness to participate is a positive prognostic indicator, she remains dependent on external support to engage in multi-step tasks, indicating a need for continued skilled intervention to progress toward her established goals.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. Before i leave for work I just want to let you know Letty had a rough weekend and in on punishment we put away the bounce house and are draining the pool- she cut her hair again, and somehow got one of my cleaning supplies while I was deep cleaning the house yesterday and smeared it all over her face, her bed her room and even in her eyes. We called poison control and she is fine. She also has torn up her books in her room. If you have any advice I would love to hear it, ik it's probably sensory seeking but how do I satisfy the sensory seeking because clearly I'm not

03-16-2026 10:38 AM	Ruben Arreola	From an occupational therapy perspective, some of the behaviors you described (cutting hair, using cleaning supplies, tearing books) don’t necessarily look like sensory-seeking behaviors. In my opinion, they may be more consistent with learned behavior, curiosity, and testing limits, which is very common at this age. While sensory needs can certainly play a role in behavior, these types of actions often occur when children are exploring cause-and-effect or when boundaries are still being learned. In situations like this, consistent expectations and natural consequences are very important. When a child makes unsafe or destructive choices, it’s appropriate for there to be consequences so they can begin to understand limits and safety. It sounds like you handled the situation thoughtfully. What we can also work on from an OT perspective is helping Letty learn more appropriate ways to engage with her environment, things like structured play activities, clear routines, and close supervision during times when she has access to materials she might explore in unsafe ways. During our sessions, I’ll continue working on self-regulation, play skills, and appropriate ways to interact with materials. If I notice sensory needs that should be addressed, I’ll definitely incorporate activities that support those as well. Thank you for keeping me informed, and please don’t hesitate to share concerns like this. It really helps me support Letty in the best way possible.
