Occupational Background History: Henry is a 3.7 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Addie, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression and adverse behaviors. Per parental report Henry tends to struggle with emotional regulation leading to hitting others, becoming aggressive and requiring max A to self regulate. Child also demonstrates difficulty with sustained attention and impulsivity leading to decreased safety awareness.

Occupational Background Summary: Henry demonstrates difficulty with fine motor skills due to limitations with object manipulation, manual dexterity and manual control evident by premature grasp patterns, spillage and difficulty with small objects limiting fine motor precision. Henry demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Henry was assessed via standardized assessment PDMS¬2, child demonstrates a gain in both subsets with a +1 and a + 10 on the raw scores on the PDMS-2 in grasping and VMI skills, now placing the child ¬2 to -3 standard deviations below his peers his chronological age of 48 months, child age equivalency scores are between 28-34 months and 19 months (+6 months). Assessment of Goals/Response to Treatment: Henry presented to session A&O and cooperative, demo’ing an eager disposition for therapeutic activities. Skilled OT services were provided to address psychosocial, cognitive, and FM skills through structured and goal-directed interventions. To target sustained attention, VMI, and task completion, Henry engaged in a clothespin letter-matching activity, promoting pincer grasp and FM coordination; he demo'ed progress in task persistence, I completing 2/3 tasks prior to attempting transition, and req. min VC to complete final task before appropriately transitioning, indicating improved attention and follow-through. During session, Henry req. mod VC on 2 occasions to identify frustration and implement coping strategies, successfully redirecting and returning to task, reflecting emerging self-regulation and emotional awareness. Psychosocial skills were further addressed through peer-based play (Candy Land), where Henry req. min–mod VC to adhere to social rules, including turn-taking and appropriate interaction, indicating continued deficits in social reciprocity and rule adherence. Overall, Henry is demo'ing measurable progress in task persistence, self-regulation, and FM skills; however, ongoing deficits in consistent attention, emotional regulation, and social-pragmatic skills persist, and he continues to benefit from skilled OT services to improve functional participation and I in age-appropriate activities.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. The address is 1510 N Zaragoza Rd, Suite B1, El Paso, TX 79936. It is very close to the current building, and I’m happy to send a photo showing its location in relation to where we are now if that would be helpful, just let me know! I’ve already checked out the space, and it has a lot of great materials and activities that I’ll be able to use during our sessions.
