Speech Background History: Elian is a 2 year, 3 month old male who was seen on the above date for an initial evaluation per referral from his pediatrician to determine his current level of functioning and whether skilled speech therapy is warranted at this time. Elian was accompanied by his mother, who provided pertinent verbal and written information referenced in this report. English is the primary language spoken in the home. Parent reported that her primary concern relates to Elian’s expressive language, noting that he currently exhibits limited verbal output and only began using single words at 2 years of age. Mother expressed that Elian has currently has approximately 20 words in his vocabulary.
Speech Medical History: Elian was born prematurely at 34 weeks gestation and required a one-month admission to the NICU. Parent also reported that Elian’s older brother experienced delayed speech development.

Speech Assessment Summary: Elian is a 2-year, 9-month old male who is demonstrating a Moderate Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Elian's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Elian receives services 2x/week for up to 60-minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Elian is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Elian's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Elian is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Elian will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Any changes to this treatment recommendation will be reported to the referring physician in the next progress summary or evaluation report whichever comes due first as per state/federal regulations. LANGUAGE: The Preschool Language Scales Fifth Edition (PLS-5) is an individually administered instrument used to assess a child's receptive and expressive language skills from birth to age 7:11. The assessment identifies receptive and expressiv language skills in the areas of attention, gesture, play, vocal development, basic concepts, language structure, integrative language, and literacy development. The PLS-5 consists of the Auditory Comprehension standardized scale and the Expressive Communication standardized scale to obtain a Total Language Score and determine if the child is presenting with a language delay or disorder. The PLS-5 was administered to evaluate Elian's auditory comprehension and expressive communication skills. - Auditory Comprehension: RS 34 ; SS 98 - Expressive Communication: RS 32 ; SS 95 - Total Language: RS 193; SS 96 Per the auditory comprehension assessment, Elian is able to follow 1-step directions given verbal, gestural, and tactile cues, engage in symbolic play and turn-taking, respond to name given verbal repetition and tactile cues, respond to inhibitory words 'no, wait, stop,' and identify common nouns , verbs, and objects given verbal repetition. He is not able to consistently identify object function, inconsistently basic body parts, verbs, or common objects with or without support, understand 'who, what (do), where' questions with or without support, or understand pronouns. Per the expressive communication assessment, Elian is able to use 2-word utterances for a variety of pragmatic functions given direct models, participate in greetings and farewells with and without prompts, and label common pictured items, tangible nouns, and base verbs (i.e., jump) given occasional support. He is not able to use more words than gestures, consistently use 2-3 words for a variety of pragmatic functions without prompts or models, use approximately 100-110 words, produce a variety of C-V combinations without support, imitate environmental and play sounds with or without support, use present progressive verb forms, plural nouns, answer basic 'who, what (do), where' questions with or without support, or consistently use pronouns. Subsequently, it is important to note that Elian’s standardized scores on the PLS-5 should be interpreted within the context of the structured and prompted support provided during testing, which may allow him to perform at a higher level than he demonstrates in everyday communication. Although his standardized scores fall within the above-average range, they do not fully reflect his current functional communication abilities. Elian is 2 years, 9 months old, and children at this age are typically expected to use several hundred words (often 300–500+) and consistently produce multi-word utterances. Currently, Elian remains difficult to understand in spontaneous speech, inconsistently uses two-word utterances for a variety of pragmatic functions, and demonstrates an expressive vocabulary of approximately 100–110 words. Given these ongoing expressive language and intelligibility concerns relative to age expectations, Elian continues to require skilled speech-language therapy to support the development of more consistent multi-word combinations, improved intelligibility, and expanded functional communication skills across environments.

After discussion, I spoke to Mom and told her that she can adjust the disclosure of information and permission to bring patient paperwork, as she is the primary parent. Mom is also going to provide the necessary legal paperwork, as Mom stated that Dad is only allowed to have the child on Saturdays and Sundays. Pending paperwork from parent.
11-24-2025	Avery Fontenot	Mom called front office stating dad had came to Elian's Therapy and took Elian from brother, that brought Elian to therapy, and left with child. Mom stated she called the cops and handled the situation but wanted to make us aware. She also inquired if there was anyway for us to turn away dad from bringing Elian to therapy and notify her if he does try to bring child to therapy. Then Shekinah stepped in to inform mom of her options since she was primary on Elian's chart. Mom will send us the required paperwork and come in to sign consent to bring/release form for her Son.
