Speech Background History: Kalei Colato is a 5-year, 7-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to concerns regarding limited language skills. Speech Assessment Summary: The Goldman-Fristoe Test of Articulation–Third Edition (GFTA-3) was administered to assess Kaleli’s speech sound production in comparison to age-matched peers as part of this re-evaluation. Kaleli is currently 6 years old. During testing, Kaleli produced 24 articulation errors, demonstrating a decrease in raw errors compared to the previous evaluation, indicating measurable progress in speech sound production. Kaleli obtained a Standard Score of 67, which falls more than 1.5 standard deviations below the mean and continues to place him within the below-average range for speech sound development. Although progress has been observed as evidenced by a reduction in total speech sound errors, Kaleli continues to present with articulation patterns that are no longer considered developmentally appropriate for his age. Observed error patterns include a frontal lisp, gliding of liquids (/r/ ? /w/, /l/ ? /w/), cluster reduction (e.g., “poon” for “spoon”), occasional final consonant deletion, and reduced accuracy with multisyllabic words. These persistent errors continue to negatively impact Kaleli’s overall speech intelligibility across both structured and conversational contexts. Mother reported increased awareness of Kaleli’s speech sound errors, noting that he is beginning to recognize when speech sounds are produced incorrectly. Mother also reported continued progress since initiation of therapy. Speech therapist has recommended that Kaleli receive further evaluation by a pediatrician and dentist to assess for possible structural or oral-motor factors, including tonsil size and potential tongue tie, which may be contributing to ongoing articulation challenges. Mother reported that this recommendation has been provided previously as well. Speech therapy remains medically necessary at this time to address Kaleli’s speech sound disorder. At six years of age, many of the error patterns currently observed are expected to have resolved. Continued speech sound errors place Kaleli at risk for reduced intelligibility with peers, teachers, and unfamiliar listeners, which may result in communication breakdowns, frustration, and reduced participation in social and academic environments. Persistent articulation errors may also negatively impact literacy development, including phonological awareness, reading, and spelling skills. Without continued skilled intervention, Kaleli remains at risk for long-term communication challenges, decreased confidence, and difficulty fully participating in classroom and social activities. Continued skilled speech therapy is warranted to improve speech sound accuracy, increase overall intelligibility, and support successful communication across home, school, and community environments.

Assessment of Goals/Response to Treatment: The client presented as alert and cooperative, demonstrating good engagement in structured therapeutic activities. Intervention continued to target the production of prevocalic /r/ and /l/ phonemes. The client achieved 30% accuracy at the single-word level given moderate verbal and visual cues to facilitate correct tongue placement. This remains a challenging area, and consistent practice is required to improve accuracy toward the established goal of 70%. Focus was also placed on reducing the phonological process of cluster reduction in word-initial consonant blends, including /sp/, /st/, /pl/, and /tr/. The client demonstrated progress in this area, achieving 40% accuracy with moderate cues for sequencing the sounds. Continued skilled intervention is medically necessary to address these articulation and phonological deficits to improve the client's overall speech intelligibility in connected speech.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office. Thank you for your time and support.
