Speech Background History: Rene Macias is a 4-year-3 month old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to limited language skills. Rene’s mother served as the informant for the evaluation. The evaluation was performed in English, as that is the primary language spoken at home. Rene’s mother reported Rene was born at 36 weeks via vaginal delivery with no complications at birth. Mother reported high blood pressure and borderline pre-clampsia during pregnancy. Mother reported no NICU stay. Mother reported Rene developmental milestones were WNL with the exception of speech. Mother reported delay in talking however was not concerned until he was not understood leading to behaviors such as hitting, kicking, scratching, and biting. Mother reported Rene is very difficult to understand and can only understand approximately 30-40% of what he is saying. Mother stated he is currently in preschool and can be very isolated from other children due to not being understood. Mother reported he is very shy and prefers to play alone.

Speech Assessment Summary: Renee is a 4-year, 9-month-old male who was re-evaluated to determine the continued need for speech therapy services. The Preschool Language Scales–Fifth Edition (PLS-5) was re-administered for comparison to previous testing . Renee’s Auditory Comprehension raw score increased to 43, with a Standard Score of 81 and percentile rank of 10, indicating improvement; however, skills remain below age expectations. His Expressive Communication raw score increased to 33, with a Standard Score of 67 and percentile rank of 1, indicating persistent expressive language deficits. His Total Language Score increased to a raw score of 148, with a Standard Score of 73 and percentile rank of 4, reflecting overall progress but continued delay. Renee demonstrates measurable progress as a result of skilled intervention, particularly in receptive language. He can now understand analogies, identify colors, understand negatives in sentences, and comprehend spatial concepts. Despite these gains, he continues to demonstrate deficits in pronouns, quantitative concepts, and consistent vocabulary identification, which impact his ability to follow directions and understand age-appropriate language. Expressively, Renee now produces 2–3+ word utterances, demonstrating progress; however, he continues to present with deficits in grammar, pronoun use, and sentence expansion, limiting his ability to effectively communicate across settings and partners. In the area of articulation, Renee demonstrated a reduction of 8 errors, indicating progress; however, he obtained a Standard Score of <55, which remains in the severely delayed range. Previously noted error patterns persist and continue to negatively impact intelligibility. Renee continues to present with a mixed receptive-expressive language disorder and speech sound disorder, which significantly impact functional communication. Although progress has been observed, skills remain below age expectations and are not sufficient for independent, effective communication. Due to parent scheduling limitations and attendance concerns, therapy frequency will be reduced to one time per week per parent request. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Our second location is on Rich Beem and Edgemere (3551 Rich Beem Blvd Ste B, El Paso, TX 79938). I do have an opening on Wednesdays at 5:30pm, but I don’t currently have a second visit available. What days and times would work best for you all? I can check with the other therapists to see if anyone has availability to accommodate the second session or possibly at our other location.

If transportation continues to be a barrier and the times we have available don't work for you, there are also some home health agencies that provide therapy at daycares. Just as a heads up, Rene’s authorization ends on 4/10, so he will be due for a re-evaluation with us here in March. Some insurance plans only cover evaluations once every 6 months, so if you’re thinking about switching providers, just let me know, so we can plan accordingly to avoid any potential coverage issues. The most important thing is that Rene continues receiving services without any interruption 😊

Rene was approved for the frequency of 2x a week by insurance. But since we are behind on 21 visits, and a re-evaluation is coming up soon, I would recommend we complete 4 visits a week to catch up as much as we possibly can. I am available Monday(8am-4 pm), Tuesday (8am-9am, 10:30am-12pm, 1pm-1:30pm, 2:30pm-4pm), Wednesday (8am-4pm) and Thursday (8am-9am, 10:30am-12pm, 1pm-1:30pm, 2:30pm-4pm) every week. Until then, I wanted to ask you if we could move his Tuesday and Thursday appointments to 6pm, and keep the Monday and Wednesday at 5:30pm. I have two brothers on my caseload that I would like to transfer back-to-back sessions during that time slot. Let me know if that works for you! :)
