Patient was seen on July 29.24 for a scheduled physical therapy evaluation to assess current level of function. Mother is not sure if feet are turning in due to weight or sedentary life. Patient weighs 102 lb at this time. Mother is trying to go out for daily walks, patient is jumping on trampoline, and enjoys using scooter. Patient will fuss to do activity but overall will perform all activity. Patient cannot coordinate jump and arm movement for jumping jacks, no arm swing or trunk rotation during running. Parental Report/Primary Concern/Goals: Parent would like to see child become more active, coordinate body, get stronger, straighten feet out so not to lose balance so much.

Without skilled physical therapy, the patient is at risk for developing further impairments that may negatively impact the ability to perform both simple and complex daily tasks, sustain proper posture, fulfill ADLs , and limit overall quality of life. Skilled physical therapy services are required to address and promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. The patient presents with hamstring tightness, lower extremity weakness, including decreased hip stabilizer strength, impaired ankle strategies, delayed protective and reflexive responses to uneven surfaces, limited activity tolerance, impaired balance and coordination, atypical gait mechanics, and decreased kinesthetic awareness. These impairments negatively affect the patient’s ability to safely and efficiently participate in age-appropriate activities of daily living (ADLs), functional routines, and gross motor play, thereby impacting overall quality of life. The patient is a 10-year-old male who scored within the 5–7 year age equivalent range in the Gross Motor domain of the Bruininks-Oseretsky Test of Motor Proficiency, Second Edition (BOT-2), indicating well below average to below average performance in both simple and complex motor skills. These results demonstrate a moderate to severe delay in gross motor development when compared to same-aged peers. Ongoing atypical gait mechanics, including bilateral inward foot progression, may contribute to abnormal loading patterns across the lower extremity kinetic chain. Over time, this may lead to compensatory movement strategies, reduced shock absorption, and increased stress at the ankles, knees, hips, and lumbar spine, placing the patient at higher risk for joint pain, overuse injuries, postural asymmetries, and decreased movement efficiency as growth and physical demands increase. Without skilled physical therapy intervention, muscle weakness, impaired postural control, limited flexibility, and decreased kinesthetic awareness may worsen, further compromising balance reactions, endurance, and motor planning. These impairments may negatively impact the development of advanced gross motor skills, delay achievement of developmental milestones, and reduce the patient’s ability to safely navigate uneven surfaces, participate in recreational activities, and keep pace with peers.
