Feeding Background History: Katerina is an 10-month-old female referred for a feeding evaluation by her pediatrician due to difficulty consuming pureed foods. Her mother, Mrs. Bertolli, and grandmother served as informants for this evaluation. Mrs. Bertolli reported that when purees were introduced between 6–9 months, Katerina did not transition easily and consistently refused these textures. Around July 2025, her mother initiated a baby-led weaning approach, at which time Katerina began to accept solid foods with more success. A primary concern was poor weight gain between 6–9 months; however, since the introduction of baby-led weaning, Katerina has gained approximately one pound. While progress has been noted, her mother continues to express concern regarding food textures, particularly as Katerina tends to eat/drink quickly and will occasionally gag or choke on certain foods. Mrs. Bertolli reported no known allergies and no history of respiratory issues. Katerina’s feeding and swallowing skills were evaluated through a bedside swallow evaluation and skilled meal observation. Due to the family’s language background and communication needs, the language of intervention will be English.
Feeding Medical History: Mrs. Bertolli reported an unremarkable pregnancy overall. Thyroid labs were slightly abnormal during the pregnancy but did not require medication and were monitored throughout. During the last trimester, low iron levels were identified and successfully managed with iron supplements. Mrs. Bertolli was also followed by a maternal-fetal specialist due to her age (38) and a long interval since her previous pregnancy (17 years); however, no complications were noted, and the baby remained healthy throughout gestation. Katerina was delivered via a planned cesarean section at 39 weeks, weighing 5 lbs 14 oz. The delivery and postpartum course were without complication, with both mother and baby discharged home after a three-day hospital stay. At this time, Katerina has no diagnosed medical conditions and no history of major illness or significant injury.

Based on the bedside swallow evaluation and skilled meal observation, Katerina presents with a CHRONIC PEDIATRIC FEEDING DISORDER and OROPHARYNGEAL DYSPHAGIA, characterized by impaired oral intake that is not age-appropriate. These difficulties may be attributed to a combination of medical history (including prior concerns for poor weight gain), nutritional status (limited food repertoire and inconsistent intake), feeding skill deficits (sensory sensitivities and decreased oral motor coordination), and/or psychosocial factors (problematic mealtime behaviors and learned responses). There are also concerns regarding the pharyngeal phase of the swallow, as Katerina demonstrated clinical signs and symptoms of possible aspiration/penetration (i.e., increased respiratory effort, watery eyes) when consuming liquid consistencies. These events may be related to her rapid pace of drinking and lack of pacing. Mrs. Bertolli denied any history of unexplained fevers or aspiration pneumonia. The Pediatric Eating Assessment Tool (PediEAT) was completed by Mrs. Bertolli as a subjective measure of Katerina’s feeding behaviors. Results indicated a rating approaching β€œConcern” on the subscale for problematic mealtime behaviors. Scores on the remaining subscale including physiologic symptoms, selective/restrictive eating, and oral processing fell within the β€œNo Concern” range. Overall, Katerina’s PediEAT results are consistent with her current diagnosis and observed performance during the feeding evaluation. CURRENTΒ FEEDING:Β Katerina currently eats with assistance from her parents, with meals typically offered in her highchair on a consistent feeding schedule. She consumes approximately 6 oz of formula three times per day and is introduced to solids three times per day. Each eating session lasts about 20–30 minutes when she accepts the foods presented, but may extend up to an hour when non-preferred foods are offered. Her mother and grandmother expressed concern regarding her limited food inventory, as she currently prefers only about eight foods, including bananas, applesauce, yogurt, sweet fruits, and rotisserie chicken from Sam’s Club. When non-preferred foods are presented, mealtimes often become stressful, lasting up to an hour, and Katerina may gag at the sight of these items. Katerina’s appetite is variable, and she demonstrates a strong preference for crunchy, lumpy, or mixed-consistency foods served warm in temperature. When presented with non-preferred items, she may gag or throw the food. ACCEPTED FOODS: Katerina currently accepts approximately 43 solid foods and 2 liquids (water and formula). Her diet includes a variety of carbohydrate sources (e.g., rice, sweet potato, potato, oatmeal, pancakes, rice-wheat cereal, spaghetti), protein sources (e.g., chicken, turkey, ground beef, ground turkey, nut butter, yogurt, eggs, cheese, broccoli soup, caldo de pollo, chicken and rice), fruits (e.g., apple, blueberries, kiwi, pear, watermelon, strawberry, pineapple, peaches/nectarines, banana, cantaloupe, orange, coconut, mango, plums), and vegetables (e.g., green beans, cauliflower, squash, carrots, peas, broccoli, cucumber, lettuce, spinach). Despite this broad food inventory, she consistently consumes only about 8 preferred foods, with regression in acceptance and increasing selectivity. According to the USDA Dietary Guidelines and the American Academy of Pediatrics (AAP), infants between 8–12 months of age typically require approximately 750–900 calories per day from a varied, nutrient-dense diet. By this stage, formula or breast milk should gradually decrease, with no more than 16 fluid ounces of formula per day, as solid foods become the primary source of nutrition. Infants are generally expected to consume three meals per day with two optional snacks of solids to support adequate growth and development. Katerina’s exact daily caloric intake is unknown; however, based on caregiver report, her limited food acceptance, prolonged mealtimes, and gagging responses to non-preferred items suggest her intake may not be meeting age-appropriate dietary expectations. FEEDING OBSERVATIONS: During the feeding observation, Katerina was seated in a highchair with a stable 90-90-90 posture (hips, knees, and ankles) throughout the session. A variety of foods were presented, including preferred items such as shredded cheese, organic melt hoops, and applesauce. She readily accepted and consumed approximately 70% of the preferred foods offered. When the preferred purΓ©e was initially presented, Katerina turned her face away and closed her mouth. With visual and verbal cues provided by her mother, she eventually opened her mouth and accepted the spoon, with her mother and grandmother using verbal praise to encourage intake. Katerina demonstrated adequate bites of the preferred foods with 1:1 supervision. In contrast, when presented with a non-preferred food (at home chicken breast), she immediately gagged, turned her head, and closed her mouth. Parent education was provided regarding the Steps to Eating Hierarchy to help the family understand Katerina’s current level of tolerance for non-preferred foods. According to caregiver report, when non-preferred foods are presented at home, Katerina may allow her parent to place the food in her mouth but will typically spit it out. She demonstrates strong texture-based aversions, particularly to wet or slimy foods. These behaviors are consistent with functioning at the early β€œtaste” stage of the hierarchy, as she may briefly allow non-preferred foods into her mouth with caregiver assistance but does not swallow them. This reflects limited oral tolerance and emerging sensory exploration. Her behaviors during the evaluation were reported by caregivers to be consistent with her typical feeding patterns at home. ORAL MOTOR SKILLS: During the observation of oral intake, Katerina demonstrated a munch-bite pattern with active lip movement and anterior protrusion/retraction of the tongue, suggesting limited dissociation of oral structures. She presented with several oral motor deficits, including reduced tongue lateralization, a vertical chewing pattern with limited rotary movement, shortened mastication time, and occasional drooling. Poor bolus formation was observed, and she displayed overstuffing behaviors without pulling or biting from larger pieces of food, which further contributed to inefficient mastication. Katerina also did not demonstrate effective spoon stripping during purΓ©e trials. Overt signs of pharyngeal dysphagia (i.e., increased respiratory effort and watery eyes) were observed during water consumption via both open cup and straw, which may be attributed to poor coordination of oral and swallow function. By approximately 11 months of age, children are typically expected to demonstrate emerging rotary chewing skills, improved tongue lateralization, and adequate bolus formation across a variety of textures. Ongoing monitoring is recommended as Katerina is introduced to increasingly complex textures during intervention. Overall, she presents with decreased oropharyngeal motor coordination and efficiency, which may be contributing to concerns regarding the safety and effectiveness of her oral intake. RECOMMENDATIONS: Feeding therapy is recommended to address Katerina’s oral dysphagia, oral motor deficits, sensory-based aversions, and regression in food acceptance. Goals will target safer, more efficient intake by addressing poor bolus formation, overstuffing, limited chewing, and swallow coordination. Intervention will focus on rapport-building, increasing tolerance of non-preferred foods through the Steps to Eating Hierarchy and food chaining, and improving oral motor skills for chewing, tongue movement, and bolus control. Parent coaching will be essential, with a home program provided to support carryover, reduce mealtime stress, and encourage safe exploration of new foods. Therapy will be conducted in English with ongoing monitoring of swallow safety.

Caregivers will play a key role in carryover through the Home Management Intervention Program (HMIP) to reduce mealtime stress and support long-term success.

Feeding Assessment Summary: Katerina is a now 16-month-old female who was initially referred for feeding therapy due to difficulty transitioning from purΓ©es to solids, concerns for poor weight gain, limited food acceptance, oral motor inefficiency, and clinical signs consistent with oropharyngeal dysphagia. She is being seen today for re-evaluation to determine whether continued skilled feeding therapy remains medically necessary. At the time of her initial evaluation, Katerina presented with CHRONIC PEDIATRIC FEEDING DISORDER and OROPHARYNGEAL DYSPHAGIA characterized by reduced tongue lateralization, immature chewing patterns, overstuffing behaviors, limited tolerance of textures, and clinical signs concerning for pharyngeal dysphagia with liquids, including watery eyes and increased respiratory effort. She relied heavily on purΓ©es and bottle feeding and demonstrated decreased bolus formation, inefficient mastication, and poor pacing with liquids. During today’s re-evaluation, Katerina demonstrated significant functional improvement in feeding and swallowing skills. She consumed applesauce via spoon with appropriate spoon stripping and functional bolus manipulation. Mild labial spillage was observed only when the spoon was overloaded; however, once the bolus entered the oral cavity, she demonstrated effective control. She was able to drink applesauce from an open cup without difficulty, indicating improved oral control and coordination. With thin liquids via straw, she demonstrated improved suck–swallow–breathe coordination, sustaining drinking for approximately 6–8 seconds before independently pausing to breathe, and up to 10 seconds on additional trials, without overt clinical signs of aspiration or respiratory distress. When presented with regular solids such as chicken and Cheetos, Katerina demonstrated closed-mouth chewing, active tongue lateralization, emerging rotary mastication, and appropriate mastication time relative to consistency. She was able to manipulate the bolus effectively, determine when additional chewing was required, and safely initiate the swallow. No overt clinical signs or symptoms of aspiration or penetration were observed during any trials presented during this session. Since initiating feeding therapy, Katerina has made substantial gains in dietary variety and feeding independence. Prior to therapy, she primarily consumed purΓ©es and bottle feeds. Currently, she accepts a wide range of proteins, starches, fruits, and vegetables, including chicken, meat, brisket, tofu, yogurt, cottage cheese, eggs, ground turkey, turkey breast, pancakes, bread, pasta, rice, mac and cheese, pizza, carrots, sweet potato, cucumber, oranges, squash, tomato, and mixed textures such as brisket combined with beans. Caregiver report confirms that she has discontinued bottle use, demonstrates flexibility with textures, and is able to manage chopped and mixed consistencies safely. Mealtimes are no longer prolonged or stressful, and intake is developmentally appropriate for age. Based on today’s clinical findings and caregiver report, Katerina no longer presents with oropharyngeal dysphagia or pediatric feeding disorder. She demonstrates age-appropriate oral motor coordination, effective bolus formation, safe swallow function, appropriate pacing with liquids, and functional feeding skills across a variety of textures. Her prior feeding challenges have resolved with skilled intervention and caregiver implementation of home strategies. At this time, continued skilled feeding therapy is not medically necessary. Katerina demonstrates safe, efficient, and developmentally appropriate oral intake. Discharge from feeding therapy is recommended. Caregivers were educated to continue offering a balanced diet, maintain appropriate pacing with liquids, and support continued exposure to a variety of textures to reinforce feeding development. Re-referral may be considered should new feeding or swallowing concerns arise in the future.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

My Pregnancy: my thyroid labs were a little off but no medications taken just monitored and low iron labs my last trimester my obgyn prescribed iron supplements and was fine.
*I Did see a maternal fetal specialist to make sure baby was ok my whole pregnancy. I'm assuming my obgyn was considering my maternal age at 38 and I had not carried a baby for 17 years so they just wanted to make sure everything was going as it should. The baby was perfectly healthy they just said she was a little bit little but normal. She weighed 5lbs 14oz . I had a planned C-section that went very well. I stayed in the hospital with the baby for 3 days and was discharged home with no issues.

Pediatrician says she has met all her milestones even some advanced fine motor skills with the exception of gross motor function being crawling and pulling up to stand alone is not met yet .(Her Pediatrician did already refer her to get an evaluation with physical therapy just to see if she needs some additional help with that.
( she does many gestures to include clapping, waving, points, smiles, laughs, puts arms up to pick her up , jumps in her jumper,hand gestures with songs, turns pages on a book, feeds herself with her hands using pincer and palmar grasp , drinks out of

I noticed that the evaluation is currently scheduled as a Feeding Evaluation, but it sounds like you may actually be seeking a Speech and Language Evaluation. There might have been an error on our end, but I’ll double-check with the front office when I’m in tomorrow to make sure we have the correct referral from the pediatrician. If there’s a mix-up, it should be a quick and easy fix. I’ll follow up with you tomorrow once I have more information!
