Eleanor's mother served as the informant for this evaluation. Per mother report, she has no concerns regarding Eleanor's receptive or expressive language, stating that she has no difficulty following directions and is talkative. Speech Assessment Summary: Eleanor is a 5 year, 6 month old female who was re-evaluated to determine continued eligibility of speech and language services. The Goldman-Fristoe Test of Articulation (GFTA-3) was used to assess her current articulation and phonological skills. The following were her scores under consonant inventory subtest: Standard Score 51 (Raw Score 53), Percentile Rank 0.1. Scores indicate a PROFOUND ARTICULATION/PHONOLOGICAL DISORDER. The following errors were noted: fronting (palatal and velar), prevocalic voicing in final position of words, gliding, and omission of final /r/ phoneme. At her age, children are expected to have mastered the majority of English consonant sounds and to demonstrate speech that is highly intelligible (approximately 90–100%) to unfamiliar listeners. Phonological processes such as fronting of velar and palatal sounds, gliding, voicing errors, and final consonant omissions are typically eliminated by approximately 3 to 5 years of age and should no longer be present. While mild or inconsistent distortions of /r/ or vocalic /r/ may still be observed in some children at this age, consistent omission of final /r/ is not considered developmentally appropriate. Eleanor’s speech sound errors are not appropriate for her chronological age and represent a significant delay in speech sound development. These error patterns should have been resolved several years earlier and are indicative of a profound articulation/phonological disorder when compared to same-age peers. The severity and variety of errors negatively impact Eleanor’s overall speech intelligibility and her ability to effectively communicate in academic and social settings. This means that all copays, coinsurance amounts, deductibles, and out-of-pocket maximums will reset.
