Speech Assessment Summary: Victoria is a 3-year, 11-month year old female who is demonstrating a Severe Expressive Language Disorder (F80.1) secondary to Childhood Apraxia of Speech (R48.2). These deficits impact Victoria's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Victoria receives services 2x/week for up to 60minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Victoria is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Victoria's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards her goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Victoria is not able to clearly or effectively communicate her needs and wants for her personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Victoria will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Any changes to this treatment recommendation will be reported to the referring physician in the next progress summary or evaluation report whichever comes due first as per state/federal regulations. LANGUAGE: The Preschool Language Scales Fifth Edition (PLS-5) is an individually administered instrument used to assess a child's receptive and expressive language skills from birth to age 7:11. The assessment identifies receptive and expressive language skills in the areas of attention, gesture, play, vocal development, basic concepts, language structure, integrative language, and literacy development. The PLS-5 consists of the Auditory Comprehension standardized scale and the Expressive Communication standardized scale to obtain a Total Language Score and determine if the child is presenting with a language delay or disorder. The PLS-5 was administered to evaluate Victoria's auditory comprehension and expressive communication skills. - Auditory Comprehension: RS 37 ; SS 84 - Expressive Communication: RS 30 ; SS 74 - Total Language Score: RS 154 ; SS 76 Per the auditory comprehension assessment, Victoria is able to follow 1-2 step directions given verbal and gestural cues, respond to name, engage in symbolic play and turn-taking, respond to name given verbal repetition and tactile cues, respond to inhibitory words 'no, wait, stop,' and identify common nouns, objects, verbs, colors, and basic body parts with and without support. She is identify object function, consistently understand 'who, what (do), where' questions with or without support, or understand pronouns. Per the expressive communication assessment, Victoria is able to use single-word utterances and/or two-word phrases (i.e., thank you, all done, more please) for a variety of pragmatic functions with and without prompts, participate in greetings and farewells, and label pictured items, tangible nouns, and verbs with verbal approximation. She is not able to use more words than gestures, consistently use single-words or phrases for a variety of pragmatic functions without prompts or models, use approximately 30-words, produce a variety of C-V combinations without support, imitate environmental and play sounds with or without support, answer basic 'who, what (do), where' questions with or without support, accurately label basic body parts, verbs, or common objects or pictures nouns without prompts, or use pronouns. ARTICULATION: A formal standardized articulation assessment, including the Goldman-Fristoe Test of Articulation–3 (GFTA-3), was not administered due to Victoria’s significantly reduced intelligibility and limited verbal output, which prevented the collection of reliable and valid responses. Her speech production is characterized by motor planning deficits consistent with Childhood Apraxia of Speech (CAS), resulting in inconsistent and difficult-to-interpret output. Per observation and parent report, Victoria was able to produce phonemes /p, b, m, w, n, d, t, k, g, h/ and most long and short vowels, and she imitated a variety of consonant–vowel (CV) combinations given moderate to maximum verbal, visual (e.g., mirror), and tactile support. It is recommended that her speech sound development continue to be monitored as verbal output increases and assessed if warranted. ORAL MECHANISM EXAMINATION: Oral-peripheral structures and function were judged to be adequate for the production of speech. Per informal observation and clinical judgment, Victoria exhibits full range of motion of the oral structures, with no signs of restriction or structural abnormality. The tongue, lips, and jaw demonstrate adequate strength and range of motion for speech production. Coordination of these structures appeared intact for non-speech (oral-motor) tasks; however, difficulties with coordination during speech tasks are addressed in the motor speech section below. Further assessment is not warranted at this time. MOTOR SPEECH PLANNING: Childhood Apraxia of Speech (CAS) is a motor speech disorder characterized by impaired planning and programming of movement sequences necessary for accurate speech production, in the absence of neuromuscular deficits. Children with CAS often demonstrate inconsistent speech errors, difficulty with sound sequencing, disrupted coarticulatory transitions, and prosodic abnormalities, which negatively impact overall speech intelligibility and functional communication. Assessment and diagnosis of CAS require a comprehensive, dynamic motor speech evaluation rather than reliance on a single standardized measure. Clinicians assess a child’s ability to imitate and spontaneously produce speech across increasing levels of complexity (e.g., vowel production, consonant-vowel syllables, reduplicated and variegated sequences, and multisyllabic words). Key diagnostic markers include inconsistency of errors, increased difficulty with longer or more complex utterances, groping behaviors, and atypical prosody. Evaluation typically incorporates both standardized tools and informal, criterion-referenced measures, with emphasis on observing motor planning breakdowns and response to cueing. The Kaufman Speech Praxis Test (KSPT) and Kaufman treatment approach provide a structured, hierarchical framework for both assessment and intervention in children with suspected CAS. The KSPT systematically evaluates a child’s speech production skills from simple to complex syllable shapes, allowing clinicians to identify the level at which motor planning breakdown occurs. This information directly informs treatment planning by guiding target selection based on developmental and motoric readiness. The Kaufman approach further supports intervention by emphasizing successive approximations, simplified word shapes, and intensive practice of functional utterances, which are critical for improving motor planning, speech accuracy, and overall intelligibility. - Accuracy Patterns: Victoria demonstrated significantly reduced accuracy across speech tasks, with an inability to consistently blend individual sounds into cohesive word forms, particularly at the CVC level. Productions were marked by errors in both short and long vowel productions, as well as difficulty sequencing consonant-vowel combinations. Overall intelligibility was judged to be less than 10% in connected speech. - Consistency: Speech productions were inconsistent across repeated trials, with variability in both consonant and vowel productions. Victoria was unable to reliably produce the same word in the same way across multiple attempts. - Support: Victoria required maximal, multi-sensory cueing to support speech production. Effective supports included verbal modeling, visual feedback (e.g., use of a mirror), and additional tactile and gestural cues. Despite intensive cueing, accuracy remained limited. - Prosody: Prosodic errors were observed, including inappropriate stress patterns across syllables. Speech production did not reflect typical rhythm or intonation patterns. - Motor Planning Observations: Victoria exhibited significant motor planning deficits, characterized by frequent groping behaviors and visible articulatory searching. There was a notable inability to plan and execute coordinated movements necessary for speech sound production, particularly as task complexity increased. Based on the above findings, Victoria presents with a severe motor speech disorder consistent with Childhood Apraxia of Speech (CAS).

Speech Rehabilitation Potential: Good. with consistent Speech Therapy and parental support. The prognosis with skilled speech-language therapy is good; without intervention, the prognosis is guarded. Victoria presents with a severe expressive language disorder secondary to speech sound disorder consistent with Childhood Apraxia of Speech (CAS), characterized by deficits in motor planning and programming that significantly impact her expressive language abilities and overall intelligibility. She requires ongoing intervention to improve her ability to produce and sequence sounds accurately, as well as to support functional communication across daily environments. Victoria will be discharged from therapy when all goals are met and she achieves standard scores above -1.5 standard deviations in all targeted areas, indicating skills within the mild to average range. Parents will be responsible for implementing Victoria’s Home Management Intervention Program to support carryover and generalization of skills. The evaluation was completed in English, and all treatment sessions will be conducted in English. It is recommended that Victoria receive speech-language therapy no less than 2 times per week for 30–45 minutes per session to maximize functional communication outcomes and ensure continued progress toward her rehabilitation goals.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Please let me know, so I can make sure to enter frequency in plan of care. However, the 2x/week would be ideal. I have not written the evaluation report yet, but she will be receiving a diagnosis of a mixed expressive/receptive language delay. As speech-language pathologists we provide diagnoses pertaining to speech, language, or feeding. The treating therapist can provide you with her plan of care once Victoria is approved for services if you need it :)

To your right, you will see 'Building A" sign, turn there and New Beginnings Building A will stand to the left. It will be fairly empty so don't be concerned!

As her language begins to expand and she develops more age-appropriate sounds, there could be the possibility she has an articulation disorder. Examples are front (i..e, tat/cat), substitutions (i.e., fumb/thumb), or cluster reduction (i.e. sing/swing), etc. It's not a given but is expected when there is a delay in language, however, each child develops different and she may not present with one! 12-16-2025 06:48 PM	Rachael Haduck	It’s called childhood apraxia of speech (CAS). It’s a neurological disorder where the brain struggles to coordinate muscle movements for speech production. Think of how I said it before, it s a misfire of neurons so we need therapy to build this connections. Victoria is very smart- she understands a lot but has that difficulty with being able to verbalize it. She has the words but cannot execute them which has what led me to believe she may have CAS- that inconsistent speech, difficultly stringing syllables together (i.e., cat [1 syllable], lady [2 syllables], dinosaur [3 syllables]), and sound distortions. This may be the reason why her expressive language is delayed. I know it feels like a lot, but with intensive speech (as we do now) and increase in oral motor planning, Victoria’s language and intelligibility (how much we understand her, 50% of the time, 20% of the time, etc) will improve. :)

SISCO (Self-Insured Services Company) is an independent third-party administrator (TPA) that often manages health plans that use the BlueCross BlueShield (BCBS) network. SISCO itself is not part of the Blue Cross Blue Shield Association. Here is how the relationship works: • SISCO's Role: SISCO is a privately held, independent TPA that handles the administrative aspects of employer-sponsored health plans, such as claims processing, benefits management, and customer support. • BlueCross BlueShield's Role: Many employers who use SISCO for administration also contract with BCBS for access to its nationwide network of healthcare providers. For example, Sysco Corporation offers its employees several medical plans that are administered by SISCO but use the BCBS network.

03-10-2026 05:03 PM	Rachael Haduck	yes. it's called the oral diadochokinesis (DDK) where a patient repeats 'puh-tuh-kuh' as fast and as accurately as they can to gage speed, coordination, and agility of the tongue, lips, and palate. you can work on it in isolation, repeating each sound 3-5x, and then 'blending' it together to say 'puh-tuh-kuh' this helps to improve speech clarity, coordination, and strengthen oral muscles.
