No overt clinical signs or symptoms of aspiration or penetration were observed, and according to caregiver's report, Andrew has no history of aspiration pneumonia or unexplained fevers. To supplement clinical findings, Andrew’s parents completed the Pediatric Eating Assessment Tool (PediEAT), a caregiver-reported measure of feeding behaviors. They reported that Andrew primarily prefers foods that are smooth, sweet, or bland, and typically prefers them served at either a warm or cool temperature. Feeding Assessment Summary: Andrew is a 5-year-3- month-old male who is being seen for a feeding re-evaluation to determine the continued need for skilled feeding therapy services. He was initially diagnosed with CHRONIC PEDIATRIC FEEDING DISORDER and OROPHARYNGEAL DYSPHAGIA characterized by limited food repertoire, sensory-based feeding aversions, reduced oral motor coordination, overstuffing behaviors, and delayed chewing skills. During this authorization period, Andrew has demonstrated measurable progress in feeding tolerance, oral motor skills, and caregiver implementation of strategies; however, persistent deficits continue to impact safe and efficient oral intake and diet expansion. Since initiating therapy, Andrew has met 5 out of 5 short-term goals, including increased engagement with novel foods through the Steps to Eating Hierarchy, improved pacing with preferred foods, improved tongue lateralization with maximum verbal and visual models, and increased caregiver carryover of feeding strategies within the home environment. Parents report that he has added new foods including oranges and ham sandwiches and has demonstrated improved bite-and-pull skills and chewing efficiency. Additionally, caregivers report he is now able to consume eggs, which were previously restricted due to allergy concerns. Despite these gains, Andrew continues to demonstrate food jagging, limited independent progression beyond early Steps to Eating levels for non-preferred foods, overstuffing behaviors, and inconsistent rotary chewing patterns. FEEDING OBSERVATION During the re-evaluation, Andrew was seated appropriately at the table and presented with both preferred and non-preferred foods. With banana (preferred), he demonstrated bite-and-pull skills and emerging diagonal chewing with intermittent closed-mouth mastication. Overstuffing behaviors were observed, and pacing strategies were required, including verbal cues and liquid wash. He required reminders to clear his mouth before taking additional bites and demonstrated intermittent food holding. Rotary chewing is emerging but not yet consistent across trials. With non-preferred items such as chicken wing and macaroni and cheese, Andrew demonstrated initial hesitation but was able to engage at higher Steps to Eating levels compared to baseline. He demonstrated bite-and-spit once with chicken wing before successfully chewing and swallowing a small portion. With macaroni and cheese, he completed one bite-chew-swallow sequence and tolerated tactile interaction, including touching the food and licking fingers at the end of the meal. He continues to require maximum verbal and visual support to advance beyond early interaction stages. Fidgeting and difficulty remaining seated during meals were noted and appear to impact sustained intake. Andrew demonstrates sensory hyposensitivity and gravitates toward strong sour and salty flavors, including pickles and lemons. He frequently describes foods as “too sour,” indicating emerging but limited descriptive language regarding taste profiles. This sensory-seeking pattern may contribute to overstuffing behaviors and rapid intake. Assessment of Goals/Response to Treatment: Andrew actively participated in today’s skilled occupational therapy session and remained engaged in all therapeutic activities presented. The session began with a therapist-led sensorimotor activity, presented as a “dinosaur hunt,” to support sensory regulation goals. Andrew required maximum physical and verbal assistance to participate in this preparatory heavy work activity and to transition into a structured tabletop task. He was able to sustain attention for approximately two minutes with frequent verbal and tactile redirection, indicating emerging progress toward the goal of maintaining attention for five minutes. During the gross motor portion of the “dinosaur hunt,” Andrew successfully completed the multi-step task, which incorporated problem-solving and gross motor coordination. He also demonstrated tolerance for peer interaction throughout the activity. To address psychosocial development, Andrew engaged in an emotion-identification task using themed images. He required maximum assistance to accurately identify emotions such as “angry” and “sad,” indicating that this skill is still developing. For the fine motor component, Andrew required maximum hand-over-hand assistance to establish a functional tripod grasp on a writing utensil. With this level of support, he was able to maintain the grasp for approximately 15–20 seconds during a visual-motor activity before needing repositioning. This reflects initial progress toward the goal of sustaining an appropriate grasp for one minute with reduced assistance.

These foods will be the same foods for a one-month span. To help Andrew's sensory system, we will stick to a color scheme (any foods that are light in color so white or yellow). Here are some ideas: protein: yogurt, ham, chicken nuggets, turkey deli, cheese sticks, cheese, eggs. starch: rice crackers, rice cakes, veggie strips, pasta, noodles, bread, english muffin, veggies: yellow squash, corn, cucumber. fruits: yellow apples, pears, pineapple, dried mango/apple/pear. Let me know if you have any questions (: However, I have noticed that during the first sessions kiddos do take some time to adjust and tend not to eat during therapy so if you think that will be the case with Andrew feel free to pick 2 non-preferred foods and 2 preferred foods (:

When we got home my husband checked with a small camera and it looked like a worm. Andrew moved so fast and would not let us check again. Andrew has been so sick we decided to wait a couple days to take him out. My husband's ENT is closed Mondays first available is tomorrow at 8 am. But Andrew has not complained about his ear. Thank God
