Assessment was completed using the Preschool Language Scales, Fifth Edition (PLS-5), in addition to clinical observation, caregiver report, and skilled therapeutic interaction. Manny participated in the evaluation with support and demonstrated the ability to engage in functional and relational play for short periods. He required moderate to maximum prompting to maintain attention and to produce verbal responses. Throughout the evaluation, Manny frequently relied on nonverbal communication methods such as gestures, nodding, and grunting to express his wants and needs. Results from the PLS-5 indicate that Manny presents with significantly delayed language skills. For auditory comprehension, Manny obtained a raw score of 18, a standard score of 54, a percentile rank of 1, and an age equivalence of 1 year and 2 months. He demonstrates strengths in responding to inhibitory words such as “no,” understanding some specific words or phrases when paired with gestures, and engaging in functional and relational play. However, he continues to have difficulty following familiar routines, identifying familiar objects from a group, identifying photographs of familiar objects, and following commands even when gestural cues are provided. For expressive communication, Manny received a raw score of 20, a standard score of 65, a percentile rank of 1, and an age equivalence of 1 year and 3 months. He is able to imitate words, engage in play for at least one minute, and produce at least one meaningful word. Despite these strengths, he demonstrates difficulty producing a variety of consonant-vowel combinations, initiating turn-taking, using at least five functional words consistently, requesting using gestures and vocalizations, demonstrating joint attention, and naming objects in pictures. Manny’s total language score yielded a raw score of 119, a standard score of 56, a percentile rank of 1, and an age equivalence of 1 year and 2 months. Manny presents with a PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. At this time, Manny presents with a profound mixed receptive-expressive language disorder, as his overall language abilities fall significantly below age expectations. At nearly 3 years of age, his language skills are comparable to those of a child between 12–15 months. This delay significantly impacts his ability to understand directions, identify common objects, communicate wants and needs, and engage in age-appropriate social interactions. Manny has demonstrated meaningful progress since his previous evaluation. He is now able to follow simple one-step directions with approximately 80% accuracy (e.g., “give me,” “clean up”), imitate environmental sounds with 40% accuracy given support, use greetings and farewells with 70% accuracy given moderate cues, and engage in play for at least one minute with the therapist. Despite these gains, he continues to rely heavily on nonverbal communication and requires significant prompting to use verbal language. Ongoing deficits in both receptive and expressive language continue to negatively impact his functional communication across environments. Based on evaluation results, continued speech-language therapy is medically necessary to address Manny’s significant delays. Without intervention, he is at increased risk for continued communication breakdowns, frustration-related behaviors, and delays in social-emotional and overall development. Manny would benefit from continued skilled speech therapy in the clinical setting to target receptive and expressive language development, increase functional communication, and support overall developmental progress.
