Feeding Background History: Ruby is a 1-year 10-month old female who was referred by her pediatrician for a clinical feeding evaluation. Ruby’s mother served as the informant for the evaluation. Ruby’s mother reported feeding concerns with her acceptance of purees and solids. She reported that she has demonstrated an interest in food and will “try” purees and solids that are presented to her, however, will mash the food briefly before spitting out the food. Ruby currently receives the majority of her nutrition from formula (NeoSure). Ruby’s mother reported that Ruby received formula via NG tube briefly before transitioning to bottles while in the NICU. Her mother reported feeding concerns when attempting to transition to purees at approximately 6 months. She reported that Ruby never accepted purees offered, however, stated that she often showed interest and played with purees. Ruby’s mother reported that she consumes 5 bottles (5oz) of formula (NeoSure) a day. In addition to this her mother reported that she offers purees or solids 2-3 times a day. She reported that Ruby will often place these foods in her mouth, briefly mash, before spitting it out. Her mother reported that Ruby has not previously received feeding therapy and reported that her pediatrician currently does not have concerns regarding her growth or nutrition.
Feeding Medical History: Ruby’s mother reported that she was diagnosed with pre-eclampsia at the end of pregnancy. Her mother underwent a c-section at 37 weeks. Ruby’s medical history consists of a diagnosis of Down syndrome. Her mother reported that Ruby was in the NICU for approximately 24 days. Ruby’s mother also reported a hole in her heart that will require corrective/repair surgery when she is older.

Feeding Assessment Summary: Ruby is a 2-year 3-month old female who was re-evaluated to determine the need for continued feeding therapy. During the course of treatment, Ruby has demonstrated an increased willingness to engage in new foods allowing for the emergence of her tolerating tastes and textures in the oral cavity. Additionally, Ruby has demonstrated improvement of oral motor skills such as the emergence of increased lateralization and emerging chewing skills (munch chew). Ruby’s mother reported that she currently consumes 6 bottles (5oz) of formula (Similac Go & Grow) a day. In addition to this, her mother reported that she continues to offer purees or solids daily. Ruby’s mother reported an increase in Ruby’s willingness to engage with and try these items, which has also been observed in feeding sessions. During the clinical feeding re-evaluation Ruby was seated in a feeding highchair to allow for 90-90-90 positioning. Ruby was presented with a saltine cracker in which she was observed to crumble with her hands and brought to mouth to “taste” multiple times. Ruby was also presented with apple slices in which she was observed to bite/much on anteriorly multiple times. Ruby was observed to taste the “juice” in many instances with no overt reactions noted. Ruby was also presented with applesauce (granny smith applesauce) in which she appeared to tolerate on her lips, however, wiped off of her tongue multiple times. Lastly, Ruby was presented with a cookie (Gerber dissolvable arrowroot cookie) in which she was observed to bring to her mouth multiple times. Ruby was also observed to place the cookie in her mouth multiple times allowing for the cookie to begin dissolving and changing consistencies. Ruby was not observed to immediately wipe off of her tongue as seen with the applesauce. The food chaining intake form was updated by Ruby’s mother in order to determine the number and types of food (if any) Ruby currently eats and accepts. It revealed that Ruby tends to prefer to explore foods that offer crunchy, crisp, and smooth textures; warm temperatures; and salty, tart, and bland tastes; and has a “good” appetite. Since Ruby is not currently eating solids, the food chaining intake form was utilized to highlight foods that Ruby has shown interest in whether it be in puree or solid form. The clinical feeding re-evaluation allowed for the observation of Ruby's increased willingness to engage with foods (purees/solids) allowing for the emergence of her tolerating tastes and textures in the oral cavity. Additionally, Ruby has demonstrated emerging oral motor skills such as increased lateralization and emerging chewing skills (munch chew). Although these improvements have been observed in the clinical feeding re-evaluation and therapy sessions, Ruby continues to appear to reject foods due to the sensory properties which does not allow for the progression of adequate oral motor skills needed for solids. Based on the clinical feeding re-evaluation, caregiver interview, and clinical observations, Ruby presents with a chronic Pediatric Feeding Disorder and oral phase dysphagia. Ruby continues to require therapeutic intervention to address oral motor skills and sensory components of feeding. Feeding therapy is deemed medically necessary to support and improve Ruby's tolerance of non-preferred textures and promote the development of age-appropriate oral motor skills. Feeding therapy is to utilize a child-led, sensory based approach, and systematic desensitization strategies to gradually increase Ruby's tolerance for non-preferred foods/textures. The progression of textures/consistencies will support increased oral sensory input and facilitate development of more efficient oral motor skills (i.e., lateralization and chewing patterns).

Feeding Progress to Date: Ruby’s progress to date is as follows: 1. In 6 months time, Ruby will engage in sensory play and increase her acceptance of new foods by tolerating looking at, touching, smelling, and/or tasting in 3 out of 5 trials in order to allow for the acceptance of new foods of varying textures/tastes. BASELINE @ 1/5 trials (vanilla yogurt) CURRENT @ 3/5 trials (touching) 2. She needs open heart surgery but isn’t a candidate because she’s so sick at the moment. They are in the OR as we speak doing a procedure to hopefully help her get to a point she gets well enough to have open heart, but the Dr said 50% chance she won't make it out of the hospital and 20% chance she won't survive the current surgery. 02-09-2026 05:06 PM	Katelyn Vera	Hello, the front office informed me that as of today I will no longer be able to see Ruby due to New Mexico/insurance credentialing. We have a therapist who is credentialed with New Mexico and will be able to see Ruby beginning this week. She will be reaching out to you soon to schedule. I am so sad that I will no longer be able to be Ruby’s feeding therapist. Ruby’s new feeding therapist is Teresa and she is an amazing therapist and Ruby will be in great hands with her. This change may also provide the opportunity for feeding sessions to be provided via telehealth in the near future. Please let me know if you have any questions.
