On the Expressive Communication subtest, she obtained a raw score of 42, a standard score of 99, and an age equivalence of 3 years and 9 months. Her Total Language Score yielded a standard score of 96 and an age equivalence of 3 years and 7 months. These results indicate that Isabella’s overall receptive and expressive language abilities fall within the average range for her chronological age. During the evaluation, Isabella presented as attentive, cooperative, and highly engaged. She demonstrated numerous strengths, including the ability to identify letters and shapes, use prepositions and possessive pronouns appropriately, explain the use of common objects, answer questions about hypothetical situations, and demonstrate understanding of pronouns, quantitative concepts, and spatial relationships. She also answered simple reasoning questions logically and used possessives accurately during structured and play-based activities. Based on standardized assessment results, parent interview, and clinical observation, Isabella DOES NOT PRESENT WITH A SPEECH OR LANGUAGE DISORDER AT THIS TIME. HER receptive and expressive language skills are age-appropriate, and she is functioning within normal developmental limits. Therefore, speech therapy is not warranted. Assessment of Goals/Response to Treatment: The patient was alert, cooperative, and participated fully in all required therapeutic tasks. Skilled intervention focused on improving motor skills, psychosocial skills, cognitive integration, and visual processing. In addressing activity initiation, the patient required minimal assistance to begin tasks in 5 out of 5 opportunities due to anxious behaviors. The use of a preparatory 'ice breaker' activity was an effective strategy to increase engagement for this session. The patient continues to work toward the goal of improved social engagement and currently requires moderate assistance to respond to conversational initiations and questions. Therapeutic activities involving matching to sample were utilized to address underlying goals, requiring the patient to sustain attention, integrate bilateral motor skills, and utilize eye-hand coordination. Continued skilled intervention is warranted to advance social engagement toward the goal of minimal support and to further develop independent task initiation.

So when you have an opportunity can you answer these questions for me: 1.) Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.) 11) Dominate Language ? English or Spanish ?

No medical diagnosis, but she was in the hospital for a week this past November for respiratory issues, she was diagnosed with RSV, Covid, but not Covid 19. She would throw up due to the phlegm and only wanted to drink milk. 3. Milestones have been met. No delays so far. 4.no sleeping or breathing Issues and gets plenty of rest. Does wake up sometimes after 6 or 7 hours of sleep when I have to go to work and will not go back to sleep until nap time. 5. No medications. Only takes vitamin gummies. 6. She goes to daycare though out the week. 7. no vision or hearing issues known. 8. I do not believe she has a speech issue. There's times I can't understand what word she's trying to say, but eventually get it. 9. May 2025 I got concern because she was rarely eating. 10. Has not been to therapy before. 11. English
