Speech Background History: Kamsi is a 5-year-old male who was seen on the above date for an initial speech and language evaluation following referral from his pediatrician to determine his current level of functioning and whether skilled speech intervention is warranted. Kamsi has a medical diagnoses of Developmental Disorder of Speech and Language, Unspecified (ICD-10: F80.9) and Autistic Disorder (ICD-10: F84.0). According to parent report, Kamsi was diagnosed with Autism in February 2025 and Kamsi has a medical history of sickle cell disease. Kamsi has not previously received speech therapy services. He currently attends O’Shea Keleher Whole Child Academy, where he has recently begun receiving occupational therapy; initiation of school-based speech therapy services are currently pending. English is the primary language in the home and academic environment. Kamsi’s mother reported concerns regarding his overall communication skills. She stated that he has significant difficulty following directions and demonstrates limited verbal output. Mother Amara, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Mother reports Kamsi struggles with ADLs such as oral care, dressing, grooming and hygiene. Mother also reports difficulty with socialization, interaction with others and difficulty following simple instructions. Kamsi demonstrates difficulty with sustained attention and impulsivity leading poor safety awareness.

Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: Kamsi transitioned to the OT room with moderate encouragement, appearing cautious yet compliant with therapist direction. He required ongoing reassurance to remain engaged and benefited from a calm, structured setting to support participation. Once in the session, Kamsi was able to sit and engage in all activities presented without complaints, following 1–2 step commands consistently. During structured tabletop and sensory-based exploration tasks, Kamsi demonstrated an initial attention span of approximately 15–20 seconds before requiring redirection. He frequently sought movement input and had difficulty sustaining focus on non-preferred tasks. With maximal verbal and visual cues, Kamsi was able to maintain attention briefly when motivated by preferred materials. A baseline assessment of upper body dressing was completed using visual sequencing cards and a simulated dressing activity. Kamsi required maximal assistance to follow each step and showed limited ability to recall or attend to the sequence of actions. He participated in a brief parallel play activity followed by a simple turn-taking game with one peer, initially avoiding social engagement and eye contact. With maximal prompting, he was able to initiate interaction and engage briefly before disengaging.

Mom never confirmed if she would schedule the visit she was only concerned about getting paper work signed by doctor. Again I tried explaining the situation to her and why we were still pending those signatures.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache.
