Parent consented to physical therapy evaluation today. Patient is a 7 year old male referred to physical therapy due to frequent falling and right foot in-toeing/pronation. Patient was seen on June 24, 2025 for a scheduled physical therapy evaluation to assess current level of function. Parent participated in PT evaluation to provided pertinent information for the assessment. Pregnancy included significant hemorrhaging in the first few weeks; mother was advised she was about to miscarriage; when she returned to see the doctor fetus had a strong heart beat; mother was placed on bed rest for 1 month; mother reports fetus was active and moving a lot. Patient passed newborn audiological screen at post birth. Currently patient is wearing glasses as needed - typically when he is tired. Glasses were prescribed due to medication. Mother reports patient is using glasses only when eyes are glitching/eye trying to adjust to see through pupil. Parental Report/Primary Concern/Goals: Parent would like to see patient’s foot not toe inward, improve strength, and improve balance especially when medication dosage is wearing off after 4 hours.

In November 2024 - mother found child limp vomiting; after starting Adderall Aug 2023 -patient was diagnosed with palpation; currently seeing cardiologist Dr Badugu 2x/year ; increase falls, losing balance when medication wears off and or not taking medication; Diagnosed with Innocent heart murmur Allergy to amoxicillin, zythromyocin

Patient/Family Education: Discussed the finding of the examination, reviewed the POC and goals with the parent. Discussed activities/strategies available to address current concerns and how to assist patient in achieving optimal developmental milestones. Parent states understanding and agrees to provide support at home in order to progress outcomes for patient. Parent provided opportunity to ask questions and all questions were answered to the parent's satisfaction.

Physical Therapy Assessment: Patient demonstrates decreased balance and mobility when medication wears off. Patient referred to physical therapy due to due to frequent falling and right foot in-toeing/pronation Patient presents with weakness, decreased balance, lacks coordination, and atypical gait which impairs patient’s ability to safely move during activities of daily living, routines and for overall quality of life. The patient is a 7 year old male who scored at 5.10 year old age equivalence with Bilateral Coordination and Balance on the BOT2 indicating Below Average status in Gross Motor skills, minimal - moderately impacting development across complex activities; with ADHD, heart palpitations impairments as additional contributing factors. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Medical Necessity: Skilled physical therapy is medically necessary to improve the patient’s current level of function, improve foot position to minimize risk for injury in the future, prevent permanent disability, improve balance, improve strength, ROM, flexibility, and promote proper posture, and gait mechanics. These interventions are essential for supporting the patient’s return to independent performance of activities of daily living (ADLs) and participation in recreational activities. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved.
