Physical Therapy Assessment: Child assessed using a Standardized Assessment Clinical Observations: head preference to left side; plagiocephaly on lateral cranium Patient demonstrates deficits in gross motor skills. Patient referred to physical therapy due to torticollis. Patient presents with weakness, decreased mobility, decreased balance, lacks coordination, decreased activity tolerance, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 1 month old female) who scored at<1 month age equivalence on the PDMS-3 assessment indicating a below average status with current level of function in Gross Motor skills; thus minimally -moderately impacting development. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Please note that we are no longer accepting the traditional Medicaid card as valid insurance on file. We require the plan-specific Medicaid insurance card, such as (Superior, Molina, El Paso Health, Wellpoint, TMHP)
If you do not have the physical card or are having trouble locating it, most Medicaid plans offer an online portal where you can access a digital E-card.
If you already have your plan’s insurance card, please email a copy to afontenot@newbeginnings-elp.com

This is Rachael, CCC-SLP with New Beginnings Pediatrics reaching out to discuss Nevaeh's speech and language development. Here are a few things we expect/see at her age, 9-months.
- Speech Sounds: babbling, use of consonants /p, b, m, n, g/, and experimenting with different pitches (like yelling), volume, and intonation
- Receptive (what we understand): responding to their name, responding to loud or unexpected sounds, understanding 'no'
- Expressive (our output, responding to what we understand): babbling, vocalizations, and gestures (pointing, hand guiding, reaching, looking between person and object); have at least one-word in their vocabulary (mama, dada)
- Social: joint attention, back-and-forth vocalization, imitation of sounds-facial expressions-actions, exploring toys (banging, shaking), eye-contact, and participating in simple games like peek-a-boo.

Red flags would be if she did not check off a majority of things listed above. What are some things she is capable of? What are you not seeing? What do you feel is emerging?

Secondary to this, we can do an evaluation and see if she qualifies. If she does not, but you still have concerns, in another 6-months (Texas regulation), she can be re-evaluated and see, again, if she qualifies :)

Overall, we have all these pre-linguistic skills (like above) we develop that are our foundation to communicating. Without those or delays in them, impair our ability to communicate. I hope that makes sense! So let's chat a bit about it and consider getting a referral from her PCP and then see about possibly moving forward with an evaluation :)
