He was initially referred by his pediatrician due to limited expressive language and difficulty communicating his wants and needs. Ezra currently receives speech and language therapy as well as occupational therapy through New Beginnings. His mother, Mrs. Castillo, served as the primary informant and reported no new medical diagnoses or significant health changes since his last evaluation. His medical history is notable for a lip and tongue tie release at two years old. Pregnancy and birth history were largely unremarkable, aside from maternal high blood pressure. Ezra experienced mild motor delays in crawling and walking but achieved other developmental milestones within expected ranges. He currently attends daycare and is not on any medications. Mrs. Castillo continues to express concerns regarding Ezra’s language development, particularly his ability to form longer sentences, understand and use verbs, and answer simple questions. She reports that while Ezra is making progress using words and short phrases, he still struggles to express himself clearly and effectively. Significant progress was noted in receptive language, specifically concerning the identification of basic body parts. When given verbal prompts, the patient correctly pointed to the corresponding body part on himself in 8 out of 10 opportunities, achieving 80% accuracy and thereby exceeding the established goal of 30%. Further gains were observed in his ability to identify objects within familiar categories. Mother was induced due to high blood pressure concerns. Child did not experience any complications at birth, does not have any visual or hearing deficits. Mother reports child struggles with self regulation leading to adverse behaviors such as biting, hitting and difficulty with transitioning from preferred tasks. Subjective: Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented to the session alert and cooperative, demonstrating readiness to engage in therapeutic activities. Skilled intervention focused on enhancing psychosocial and cognitive integration skills through cooperative play with multiple-part toys. To address task initiation, the patient successfully initiated 2 out of 4 nonpreferred task components with 4-5 verbal cues and moderate tactile prompting, showing emerging progress from a baseline requiring maximum assistance. During the structured play activity, the patient demonstrated notable improvement in attention, sustaining focus for intervals of 15-20 seconds with moderate verbal cueing to redirect, which is a significant increase from his previous 5-10 second attention span. Progress was also observed in self-regulation and social skills; the patient participated in a 3-minute turn-taking and sharing activity with one other peer, requiring 7 verbal cues to maintain engagement and appropriate social reciprocity. This marks a positive step forward from his baseline of self-isolation and aggressive behaviors, as no aggression was noted during today's interaction. Furthermore, the patient utilized a visual "Zones of Regulation" check-in chart at the start of the session with moderate verbal and gestural assistance to identify his emotional state. He continues to require maximum assistance to utilize self-regulation tools at the conclusion of tasks but is showing improved tolerance for the process. The patient is making measurable gains toward his goals, and continued skilled occupational therapy is medically necessary to improve functional independence in task engagement and social participation.

Mother was provided with information on community resources that could help supplement his language development and support continued progress. She expressed her decision to continue services once weekly, with her preferred schedule being Wednesdays at 4:00 PM. Mother was advised to keep the clinic informed of any future changes in his schedule or service needs.

If he qualifies for speech therapy services at school he can still receive private services, it is only beneficial for him. Early intervention is always the best option if possible. If so we can continue services until it’s no longer active. If not we would have to process a discharge for Ezra. When you have the new insurance we would then start the process over with an initial evaluation. It was a pleasure working with Ezra and seeing him thrive!
