His mother served as the primary informant and provided reliable background information. Per parent report, there have been no medical, household, or environmental changes since the previous evaluation. Maximus is not currently taking any medications and has no known history of major illness or injury. No vision or hearing concerns have been reported. An autism evaluation is currently pending and scheduled for January 2026. Maximus has a history of early intervention services and continues to present with significant communication delays. His mother reports observable progress since beginning therapy, including increased independence at home, improved ability to follow directions, increased responsiveness to caregivers, improved attention, and a noticeable decrease in tantrums. She has also observed an increase in Maximus’s desire to communicate and emerging pre-linguistic skills. During this period, Maximus spontaneously produced the word “diaper”; however, his mother reports that verbalizations remain sporadic, inconsistent, and not yet functional for daily communication. Based on her observations, she is requesting continuation of speech therapy services at a frequency of two 30-minute sessions per week, noting that Maximus demonstrates greater gains when therapy is provided twice weekly. The evaluation consisted of a parent interview, clinical observation, and standardized testing using the Preschool Language Scale, Fifth Edition (PLS-5), administered in English. Due to Maximus’s age and limited engagement, results were interpreted with consideration of parent report and observed behaviors. During the evaluation, Maximus demonstrated limited engagement with the clinician, primarily engaging in independent play with inconsistent eye contact and reduced reciprocal interaction. He required frequent support to attend to activities and participate in structured tasks. On the PLS-5, Maximus received an Auditory Comprehension raw score of 8, corresponding to a standard score of 50, a percentile rank of 1, and an age equivalence of 4 months. Areas of difficulty included inconsistent response to his name, limited ability to interrupt activity when called, difficulty following caregiver pointing or joint attention cues, limited anticipation of routine events, reduced object permanence skills, and difficulty understanding simple verbal requests such as “come here.” Expressively, Maximus demonstrated emerging strengths such as responding to speakers with smiling, using varied pitch and volume in his cries, and vocalizing pleasure and displeasure. Per parent report, he has produced one spoken word (“diaper”); however, verbal output remains inconsistent and nonfunctional. Expressive language weaknesses included limited purposeful vocalizations, minimal use of gestures or vocalizations to request or protest, limited imitation of actions or facial expressions, reduced attempts to seek attention from others, and inconsistent production of different vowel sounds. Based on standardized testing results, clinical observation, and parent report, Maximus presents with a PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. Although measurable progress has been observed since the previous evaluation, his receptive and expressive language skills remain significantly delayed and continue to impact his ability to communicate wants, needs, and responses effectively. These deficits place him at risk for continued communication, social, and functional challenges without ongoing intervention. Speech-language therapy is medically necessary for Maximus due to the severity of his receptive and expressive language deficits and the functional impact on his ability to communicate. Skilled speech-language intervention is required to develop foundational communication skills, increase functional expressive language, support joint attention and early interaction skills, and reduce communication-related frustration and maladaptive behaviors. Continued therapy at a frequency of two 30-minute sessions per week is warranted based on the severity of impairment, the critical period of language development, and documented progress with increased therapy frequency. With consistent intervention and caregiver involvement, Maximus’s prognosis for continued improvement is fair to good.

Assessment of Goals/Response to Treatment: The patient presented with variable attention to task, requiring frequent verbal and visual cues for redirection to therapeutic activities. In the area of expressive language, progress was noted toward the goal of producing purposeful vocalizations. The patient successfully produced two different early word approximations, such as 'ah' and 'eh , within play-based activities, meeting the established criterion of 2 out of 10 opportunities (20%). These productions were observed following clinician modeling and periods of expectant delay. However, the patient has not yet demonstrated progress towards the objective of imitating environmental sounds. Attempts to elicit vocal imitation of sounds such as animal or vehicle noises were unsuccessful at this time, resulting in 0% accuracy across presented opportunities. Continued focus will be placed on developing joint attention and foundational vocal play skills to support this goal. Finally, emerging progress was observed in functional social communication. No complications in pregnancy and birth he was actually my longest pregnancy, I did see a high risk doctor for my bleeding disorder I have and for depression and anxiety. 2. No major illnesses or accidents 3. He's delayed in his speech but I don't know if using a pacifier made a impact in this. I currently just took it away 4. He sleeps 9-10 hours at night, does sleep with his mouth closed and no snoring 5. No medications 6. He stays home with me 7. No vision or hearing issues 8. Im just concern with his speech I did do the mistake in giving him a pacifier and with my other kids I never had a issue with them speaking never did I introduce them a pacifier I don't know if thats a contribution to why he don't talk as much because when he don't have it he does try to babble and he does say a few words but not always. He does not point to objects. Eye contact is very small or he wont keep eye contact for long. He can walk very well almost speed walk hes trying he loves to explore everything and look at everything and see what things do. He understands sign language for milk and all done and more trying to teach him different ones but hes only gotten those. He's very independent.

Okay so I didn't find exactly how this doctor structures her evaluations, but it should be similar to this: 1) background information, 2) parent questionnaire and 3) play observation. So in the first part, they will talk to you about Max's development, so questions about pregnancy and birth, his sleeping patterns, his milestones, his play skills, any familial history of Autism, or any routines or repetitive behaviors that Max likes. Then the parent questionnaire will have questions like "does your child point to show interest?", "Do they bring items to show you?", or "Does he respond to his name?" Then they will score that questionnaire with a pass or fail. Then they'll just watch him play and see how he interacts with the toys, with you and with them, how he uses gestures, if he imitates actions, and how he engages in general. You can go in with a list of questions you might have for them, or write down you concerns WITH his strengths. Then you can let them know that he's already getting speech therapy and that you are interested in occupational therapy, too. I also found a link that you can read that describes how the evaluation will be! Just remember it won't change who he is, only will show us what support may help him thrive! You are such a great momma! <3

So its a step by step process and they had to see him first like another doctor to see if he is going to be eligible for a further screening for it. She asked me questions, And she played with him for about 15 mins and she told me she does see signs of autism and that now he moves on to step 2 which is a whole hr play appt with the actual doctor. Till april 7 for that one and april 22 for results there they going to talk to me what level he has it and all that comes with this diagnosis 🙏🏼😊
