Speech Background History: Quinn Page is a 3-year 7-month-old female who was referred by her pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Quinn’s mother served as the informant for the evaluation. The evaluation was performed in English, as this is the primary language at home, however it should be noted mother stated Quinn hears and understands native language (Phillippen). Mother reported Quinn was full term born via vaginal delivery and reported no complication during or after birth. No medications were reported. Mother stated Quinn gets along really well with other kids and tries to communicate with them however does not get her point across due to being timid. During evaluation Quinn was very shy at the beginning of the session and as the session progressed she warmed up and engaged in evaluation. Mother reported she is very hard to understand and is very soft spoken. Parents' main concerns are limited communication and difficulty understanding her message.

Speech Assessment Summary: The Preschool Language Scales–Fifth Edition (PLS-5) was administered to evaluate Quinn’s receptive and expressive language development. At the time of this re-evaluation, Quinn is a 4 year 1 month old female. Quinn’s Auditory Comprehension standard score is 73, indicating performance in the below average range when compared to age-matched peers and consistent with a severe receptive language disorder. Although receptive language skills remain significantly delayed, Quinn has demonstrated measurable progress during the current authorization period, as evidenced by improved raw scores and acquisition of new receptive language skills. Quinn’s receptive language strengths now include understanding the use of objects, understanding spatial concepts, understanding analogies, recognizing actions in pictures, identifying body parts, engaging in symbolic play, and making inferences. These gains reflect skills that were not previously demonstrated and indicate positive response to intervention. Despite this progress, Quinn continues to present with difficulty understanding quantitative concepts, understanding negative constructions within sentences, and comprehending sentences with post-noun elaboration. These receptive language deficits continue to impact her ability to follow increasingly complex directions and fully comprehend spoken language in daily environments. Ongoing receptive language weaknesses remain a safety concern, as difficulty understanding multi-word directions and sentence structures may place Quinn at risk in functional situations. Quinn’s Expressive Communication standard score is 64, with an increased raw score of 28, placing her more than two standard deviations below the mean when compared to age-matched peers and indicating a severe expressive language disorder. Quinn has made notable gains in expressive language during the authorization period. She is now able to name objects in photographs and produce 2–3 word utterances when provided with cues to slow her rate of speech. She continues to demonstrate strengths in seeking attention from others, using gestures and vocalizations to request items, demonstrating joint attention, and attempting verbal communication to express wants and needs. Despite these improvements, Quinn continues to present with significant expressive language delays. She continues to have difficulty independently using multi-word utterances to communicate wants and needs, using a word for a wide variety of communicative functions, labeling familiar vocabulary consistently, and expanding expressive vocabulary to age-expected levels. Overall intelligibility continues to significantly impact Quinn’s ability to be understood by both familiar and unfamiliar listeners, further limiting her functional communication. Due to limited verbal output and reduced intelligibility, an articulation screener was administered. Results indicate the presence of multiple phonological processes, including final consonant deletion, stopping, backing, and difficulty with multisyllabic words. These speech sound errors further reduce Quinn’s intelligibility and compound her expressive language deficits, making it difficult for her to effectively communicate her thoughts, needs, and ideas. Quinn’s Total Language standard score continues to fall within the severe range, indicating a severe mixed receptive and expressive language disorder. At 4 years of age, a child is expected to use sentences of four or more words, answer and ask simple WH-questions, use age-appropriate grammar (including plurals, pronouns, and basic sentence structures), and be understood by unfamiliar listeners most of the time. Receptively, a child this age should follow multi-step directions, understand quantitative and descriptive concepts, comprehend more complex sentence structures, and demonstrate understanding of increasingly abstract language. Quinn does not yet demonstrate these age-appropriate communication skills. Based on formal assessment results, parent report, and clinical observation, continued skilled speech-language therapy is medically necessary. Without ongoing intervention, Quinn remains at risk for continued delays in communication, reduced intelligibility, safety concerns related to difficulty understanding verbal directions, and challenges with social interaction and early academic readiness. Her receptive and expressive language deficits, combined with phonological speech sound errors, significantly impact her ability to function effectively within home, school, and community environments.

Assessment of Goals/Response to Treatment: The patient presented as alert and cooperative, demonstrating good engagement in therapeutic activities. The session focused on improving receptive language skills, specifically targeting the patient's ability to answer age-appropriate WH-questions. When presented with questions ('who,' 'what,' 'where') related to familiar pictures and clinician-led activities, the patient responded with 38% accuracy. This level of performance was achieved with the provision of moderate verbal and visual cues, such as a field of two picture choices and phonemic prompts to facilitate a correct response. While this data indicates an emerging understanding of basic question forms, it falls below the established goal of 70% accuracy. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
